血清降钙素原、炎症细胞因子及超敏C-反应蛋白联合检测放射性肺炎合并肺部感染的诊断价值

Diagnostic value of serum calcitonin,inflammatory cytokines and high sensitivity C-reactive protein in the diagnosis of radiation pneumonia complicated with pulmonary infections

目的探究血清降钙素原(PCT)、炎症细胞因子及超敏C-反应蛋白(hs-CRP)联合检测对放射性肺炎合并肺部感染的诊断价值。方法选取2015年1月-2016年12月于医院收治的放射性肺炎合并肺部感染的患者55例,作为试验组;选取放射治疗后同时间段但未发生放射性肺炎合并肺部感染的患者50例,作为对照组;所有患者空腹抽取静脉血,测定两组患者血清降钙素原(PCT),肿瘤坏死因子-α(TNF-α),白介素-6(IL-6),白介素-8(IL-8)及hs-CRP的含量。结果试验组患者血清PCT、hs-CRP及炎症因子(TNF-α、IL-6、IL-8)的水平均高于对照组(P<0.05);血清PCT、hs-CRP和炎症因子联合检测对放射性肺炎合并肺部感染诊断的灵敏度、特异性、准确率均优于单独检测(P<0.05)。结论联合检测血清PCT、炎症细胞因子及hs-CRP能够提高放射性肺炎合并肺部感染诊断的灵敏性、特异性及准确性,具有一定的临床意义。

OBJECTIVE To investigate the diagnostic value of serum calcitonin,inflammatory cytokines and high sensitivity C-reactive protein in the diagnosis of radiation pneumonia complicated with pulmonary infections.METHODS A total of 55 cases of patients with radioactive pneumonia complicated with pulmonary infections admitted in our hospital from Jan.2015 to Dec.2016 were selected as observation group,and 50 cases of patients with radiotherapy and without radiation pneumonia at the same period of time were selected as control group.All patients were taken venous blood when they were fasting,and the levels of serum procalcitonin（PCT）,tumor necrosis factor-α（TNF-α）,IL-6,IL-8 and high sensitivity C-reactive protein（hs-CRP）of the two groups were detected.RESULTS The levels of serum PCT,hs-CRP and inflammatory cytokines（TNF-α,IL-6 and IL-8）in observation group were significantly higher than those in control group（P〈0.05）.The sensitivity,specificity and accuracy of combined detection of serum PCT,hs-CRP and inflammatory cytokines in patients with radioactive pneumonia complicated with pulmonary infection were significantly better than that of that of individual detection（P〈0.05）.CONCLUSIONCombined detection of serum calcitonin,inflammatory cytokines and high sensitivity Creactive protein can significantly improve the sensitivity,specificity and accuracy of radiation pneumonia complicated with pulmonary infections,which has certain clinical significance and worthy of promotion.
