幽门螺杆菌感染与青光眼的相关性研究

Correlation between Helicobacter pylori infections and glaucoma

目的探讨幽门螺杆菌感染与青光眼的相关性。方法选择2014年1月-2015年12月医院眼科原发性开角型青光眼患者60例为开角型青光眼组、原发性闭角型青光眼患者60例为闭角型青光眼组、性别及年龄相匹配的健康体检者60名为对照组;采用^(14)C-尿素呼气试验测量幽门螺杆菌浓度,采用胶体金标记免疫斑点渗滤法测定血清幽门螺杆菌免疫球蛋白G(IgG),采用光学相关断层成像测定视网膜神经纤维层厚度。结果开角型青光眼组幽门螺杆菌阳性率88.33%高于闭角型青光眼组35.00%和对照组31.67%(P<0.05);开角型青光眼组血清幽门螺杆菌IgG阳性率85.00%高于闭角型青光眼组33.33%和对照组28.33%(P<0.05);开角型青光眼组和闭角型青光眼组大视盘、中视盘、小视盘视网膜神经纤维层厚度均低于对照组(P<0.05),开角型青光眼组大视盘、中视盘、小视盘视网膜神经纤维层厚度低于闭角型青光眼组(P<0.05);开角型青光眼患者幽门螺杆菌浓度与青光眼患者视盘视网膜神经纤维层厚度呈负相关(P<0.05)。结论原发性开角型青光眼患者幽门螺杆菌感染率高,幽门螺杆菌可能和原发性开角型青光眼的发病有关。

OBJECTIVE To investigate the correlation between Helicobacter pylori infections and glaucoma.METHODS A total of 60 patients with primary open angle glaucoma were selected as open angle glaucoma group,60 patients with primary closure angle glaucoma were selected as closure angle glaucoma group,and 60 healthy were selected as control group from Jan.2014 to Dec.2015.The concentration of H.pylori was measured by ^（14） Curea breath test,the serum H.pylori IgG was determined by colloidal gold labeled immunofluorescence diafiltration,and the thickness of retinal nerve fiber layer was measured by optical correlation tomography.RESULTS The positive rate of H.pylori in open angle glaucoma group（88.33%）was significaently higher than that in closure angle glaucoma group（35.00%）and control group（31.67%）（P〈0.05）.The positive rate of serum H.pylori IgG in open angle glaucoma group（85.00%）was significaently higher than that in closure angle glaucoma group（33.33%）and control group（28.33%）（P〈0.05）.The thickness of retinal nerve fiber layer in the retina was significantly lower than that in the control group（P〈0.05）.The large optic disc retinal nerve fiber layer thickness,medium optic disc retinal nerve fiber layer thickness and small optic disc retinal nerve fiber layer thickness of open angle glaucoma group and closure angle glaucoma group were significantly lower than that in the control group（P〈0.05）.The large optic disc retinal nerve fiber layer thickness,medium optic disc retinal nerve fiber layer thickness and small optic disc retinal nerve fiber layer thickness of open angle glaucoma group was significantly lower than that in closure angle glaucoma group（P〈0.05）.The concentration of H.pylori in open angle glaucoma patients was negatively correlated with the thickness of retinal nerve fibers in patients with glaucoma（P〈0.05）.CONCLUSIONThe H.pylori infection rate of patients with primary open angle glaucoma is high.H.pylori may be associated with the pathogenesis of primary open angle glaucoma.
