轻量型网片在腹股沟疝三明治式无张力修补术中应用效果及并发症影响研究

Effect of lightweight mesh on inguinal hernia sandwich-based tension-free repair and its effect on complications

摘要 目的研究轻量型网片在腹股沟疝三明治式无张力修补术中应用效果及并发症影响。方法选取2013年1月至2015年6月,常熟市梅李人民医院行非腹腔镜下无张力修补术治疗的各类腹股沟疝156例患者的临床资料。随机将156例患者分为2组,采用轻量型网片的治疗组78例和采用重量性网片的对照组78例。2组均由同一组医师行三明治式无张力修补术,术后随访12个月,对比2组的治疗效果。结果在术后随访的12个月内,治疗组和对照组出现术后复发的人数同样为0;治疗组在术后第3和第6个月出现术后疼痛诉求的患者占比17.95%和7.69%,显著低于对照组的51.28%和21.79%,差异有统计学意义(P<0.05),在术后第1和第12个月的对比差异无统计学意义(P>0.05);治疗组出现术后疼痛的患者在第3和第6个月的疼痛评分为1.58±0.41和0.89±0.61,显著低于对照组的1.88±0.29和1.71±0.25,差异有统计学意义(P<0.05),在术后第1和第12个月的对比差异无统计学意义(P>0.05);2组患者在术后并发症的发生率情况对比,差异无统计学意义(P>0.05)。结论轻量型网片能够较好的满足腹股沟疝三明治式无张力修补术的要求,在手术中取得了较好的效果且不会影响术后并发症的发生情况。 Objective To study the effect of light mesh on inguinal hernia sandwich-based tension- free repair and its effect on complications. Methods From Janual3J 2013 to June 2015, there were 156 cases of inguinal hernia treated by non-laparoseopic hernia repair in Meili people's hospital. Of these, the patients were randomly divided into two groups: 78 patients in the treatment group with lightweight mesh and 78 in the control group with normal weight mesh. The two groups were treated by the same group of surgeons with sandwich tension-free repair, followed up for 12 months. The treatment effects of the two groups were compared. Results In the 12 mouths after follow-up, none of recurrence was found in the treatment group and the control group. In the treatment group, the proportion of patients with postoperative pain complaint was 17.95% and 7.69% in the third and sixth months after operation, respectively, which were significantly lower than those in the control group (51. 28% and 21. 79% ) , the difference was statistically significant. There was no significant difference in terms of postoperative pain between the first month and the 12th month after operation. Of the patients in the first 3 months and 6 months, the pain score was 1.58 ± 0. 41 and 0. 89 ± 0.61 in treatment group, significantly lower than those in the control group 1.88±0.29 and 1.71 ± 0.25, the difference was statistically significant. And the difference in the first month and 12 months was not statistically significant. The difference in the incidence of postoperative complications was not statistically significant between the two groups. Conclusion Lightweight mesh can better meet the requirements of inguinal hernia tension-fi'ee repair and achieve good results in operation, and it does not affect the occurrence of postoperative complications.
