丽水地区幽门螺旋杆菌感染率与耐药率分析

目的分析丽水地区幽门螺旋杆菌（Hp）感染情况及其药敏谱，为临床防治Hp感染提供参考依据。方法回顾性分析2010年1月至2016年12月9126例患者的临床资料，分析Hp总体感染特征。同时对2016年105例患者和2010年201例患者药敏结果进行比较，分析本地区Hp的感染趋势。结果2010年-2016年本院Hp检测阳性率呈逐年上升趋势；不同年龄段Hp感染分布显示，20~29、30-39、40～49、50～59哕感染率分别为34．6％、32．9％、3055％、33．10％，高于其他年龄段，差异有统计学意义（P〈0．05）。2016年药敏实验显示Hp对甲硝唑耐药率较高，耐药率为85．7％、其次为克拉霉素和左氧氟沙星，耐药率分别为21．9％、31．40％。结论本地区Hp感染主要分布在20-60岁年龄段，由于胃癌的发病正在年轻化，应高度重视青壮年幽门螺旋杆菌感染。Hp感染者，不能仅当胃肠消化道疾病治疗，应当做感染性疾病，根据药敏试验结果治疗，可提高根除率，降低耐药率。

Objective To investigate the distribution of Helicobacter pylori and drug resistance in the people of Lishui so as to provide references for early diagnosis and treatments. Methods The clinical data of 9126 patients who examed in the hospital from January 2010 to December 2016 were retrospectively analyzed, then the total distribution of Helicobacter pylori infections were investigated. At the same time, the susceptibility of 105 patients were compared in 2016 and the infection trend of Helicobacter pylori of 102 patients were analyzed in 2010 in this region. Results Hehcobacter pylori positive rate showed increasing trend from 2010 to 2016. The results showed that relatively high infection rates were among 20 ~ 29, 30 ~ 39, 40 - 49, 50 ~ 59 than others. Infection rates were 81.60%, 74.40%, respectively. The differences were significant （ P〈0.05 ） . The drug resistance rate highest of Metronidazole was 85.7%, followed by clarithromycin and levofloxacin. The resistant rates were 21.9% and 31.40%. Conclusion Helicobacter pylori infection in the region is mainly distributed in the 20 to 60 age group, due to the incidence of gastric cancer is young, should pay more attention to young adults Helicobacter pylori infection. Helicobacter pylori infection can not only treat as gastrointestinal disease, they should be treated as infectious diseases, according to drug sensitivity test results can improve the eradication rate and reduce the rate of drug resistance.
