经桡动脉行冠状动脉介入诊疗中桡动脉痉挛的发生及其预测因素分析 被引量:4

摘要 目的分析经桡动脉行冠状动脉介入诊疗中桡动脉痉挛的发生情况以及其预测因素。方法选取我院接受桡动脉介入治疗的冠状动脉粥样硬化性心脏病患者84例作为研究对象,根据患者治疗过程中是否出现桡动脉痉挛将84例患者分为对照组与观察组,对照组40例患者在治疗过程中未发生桡动脉痉挛,观察组44例患者在治疗过程中出现桡动脉痉挛症状,对比观察经桡动脉行冠状动脉介入诊疗中桡动脉痉挛发生的影响因素。结果相比于对照组,观察组发生桡动脉痉挛患者的女性占比较大,且存在疾病史的患者出现桡动脉痉挛的概率较高,P<0.05,组间比较差异存在统计学意义。桡动脉痉挛发生的独立因素包括患者的临床资料、桡动脉路径异常、桡动脉直径与导管外径比值、导管交换次数、桡动脉直径与身高比值。结论经桡动脉行冠状动脉介入诊疗过程中对桡动脉痉挛的独立因素进行控制,可以有效降低桡动脉痉挛的发生,提高治愈效果。 Objective To analyze the occurrence and predictors of radial artery spasm in transradial coronary intervention. Methods 84 patients with coronary atherosclerotic heart disease who underwent radial artery interventional treatment in our hospital were selected as the study subjects. According to whether radial artery spasm occurred during the course of treatment, 84 patients were divided into control group, and observation group. The symptoms of radial artery spasm was not observed in 44 patients in control group during treatment. The symptoms of radial artery spasm occurred in 44 patients in the observation group during the course of treatment. The influencing factors of radial artery spasm during the interventional treatment of radial artery through coronary artery were compared. Results Compared with the control group, the incidence of radial artery spasm in the observation group was higher than that in the control group. The incidence of radial artery spasm was higher in the patients with history of disease (P 〈 0.05). Radial artery spasm independent factors include clinical data, radial artery pathology, radial artery diameter and catheter diameter ratio, the number of catheter exchange, radial artery diameter and height ratio. Conclusion The independent factors of radial artery spasm during the diagnosis and treatment of transradial coronary artery interventional therapy can effectively reduce the occurrence of radial artery spasm and improve the curative effect.
