黄体酮胶囊配合心理治疗对妊娠期黄体不足所致先兆流产的作用及机制探讨

Efficacy of Progesterone Capsules Combined with Mental Intervention on Threatened Abortion Due to Luteal Insufficiency and Investigation of Machanism

目的:探讨黄体酮胶囊配合心理治疗对妊娠期黄体不足所致先兆流产的效果及对血清中白细胞介素(IL)-17水平的影响。方法:纳入黄体不足引起先兆流产病例102例,随机分为治疗组51例和对照组51例。对照组给予地屈孕酮片。治疗组给予黄体酮胶囊,口服,2次/d,100mg/次;并实施心理干预。两组治疗3周后进行检测指标和疗效分析。比较两组腰酸缓解、止血及腹痛缓解时间和临床疗效。检测两组血清中孕酮(P)和白细胞介素(IL)-17水平。结果:治疗组的腰酸缓解、止血及腹痛缓解时间显著少于对照组(P<0.01)。治疗后2周和3周,治疗组血清中P含量均明显高于对照组(P<0.01)。治疗组总有效率为98.04%,显著高于对照组的84.31%(P<0.05)。治疗后2周和3周,治疗组血清中IL^(-1)7含量显著高于对照组(P<0.01)。结论:黄体酮胶囊配合心理治疗对妊娠期黄体不足所致先兆流产的作用显著于地屈孕酮疗法。

Objective：To investigate efficac of progesterone capsules combined with mental intervention on threatened abortion due to luteal insufficiency and the influence on serum level of interleukin （IL）-17.Methods： 102 patients with threatened abortion due to luteal insufficiency were randomly divided into control group （51 cases） and treatment group （51 cases）.Control group was orally treated with dydrogesterone tablets.Patients of treatment group was orally given with progesterone capsules （100mg/time and bid）,and mental intervention.Indexes and efficacy were analyzed after 3 weeks.Time of lumbago remission,hemostasis,bellyache remission and efficacy were compared in both groups.Serum level of progesterone （P） and IL-17 were detected in two groups.Resuhs：Time of lumbago remission,hemostasis,and bellyache remissionof treatment group was obviously shorter than control group （P〈0.01）.P serum level of treatment group was obviously lower than the corresponding period level of control group after 2 week and 3 week（P〈0.01 ）.Efficacy rate of treatment group was 98.04%,which was evidently higher than control group as 84.31%（P〈0.05）.After 2 week and 3 week, serum level of IL-17 of treatment group was more than those of control group （P〈0.01）.Conclusion：Efficacy of progesterone capsules combined with mental intervention in treating threatened abortion due to luteal insufficiency is obviously superior to Diprogesterone treatment.
