摘要
目的探讨急性缺血性脑卒中后抑郁症与血清尿酸、25-羟维生素D水平的相关性。方法收集连云港市第一人民医院和连云港市东方医院神经内科收治的初发急性缺血性脑卒中患者144例,于入院24h内分别检测血清尿酸、25-羟维生素D水平,在发病2周~2个月间参照卒中后抑郁症诊断标准将患者分为卒中后抑郁组和非卒中后抑郁组,统计分析两组间血清尿酸、25-羟维生素D水平的差异性,并探讨抑郁程度与血清尿酸、25-羟维生素D水平的相关性。结果卒中后抑郁组的血清尿酸、25-羟维生素D水平低于非卒中后抑郁症组的血清尿酸[(291.23±68.47)μmol/L vs.(351.13±106.00)μmol/L]、25-羟维生素D[(23.91±11.06ng/ml vs.(32.50±10.63)ng/ml]水平,差异有统计学意义(P<0.01);进一步行相关分析发现抑郁程度与血清尿酸、25-羟维生素D的水平均呈负相关并且差异有统计学意义(r=-0.347,P=0.009;r=-0.469,P<0.01)。结论急性缺血性脑卒中后抑郁症与血清尿酸、25-羟维生素D水平存在相关性,血清尿酸、25-羟维生素D水平低的缺血性脑卒中患者可能更容易患有脑卒中后抑郁症。
Objective To investigate the correlation between serum uric acid or 25 - hydroxy vitamin D level and acute ischemie post- stroke depression. Methods Totally 144 patients with the initial acute ischemic stroke from the department of Neurology of the First Peoples Hospital and the Oriental Hospital of Lianyungang City were involved in this study.The serum uric acid and 25- Hydroxy vitamin D were measured in the first 24hours,and the patients were divided into post- stroke depression group and non- post- stroke de- pression group by the PSD diagnostic criteria from 2 weeks to 2 months.The clinical data in two groups were compared respectively with test and the association between serum uric acid and 25 - hydroxy vitamin D level and the degree of depression were analyzed.Results The level of serum uric acid(291.23 ± 68.47μmol/L V.S 351.13 + 106.00μmol/L) and 25 - hydroxy vitamin D (23.91 ± 11.06ng/ml V.S 32.50 ±10.63ng/ml) in pa- tients with post- stroke depression was significantly lower than that in non post- stroke depression patients, and the difference was statistically significant (P〈0.01 _. And the degree of depression was negatively related with the level of serum uric acid and 25= hydroxy vitamin D (r = - 0.347,P = 0.009; r = - 0.469,P〈0.01). Conclusion Serum uric acid and 25- hydroxy vitamin D affecte the occurrence of PSD,and the patients with ischemic stroke at low level of them may be more prone to PSD.
