摘要
目的探讨鼻内镜下应用自体部分筛骨垂直板移植修复鼻内镜术中鼻中隔黏膜损伤的临床效果。方法 2012年1月至2017年1月在我院耳鼻喉科就诊的28例鼻中隔偏曲合并鼻窦炎、鼻息肉的患者,其中男性17例,女性11例,年龄21~68岁,平均年龄34岁。行鼻内镜下鼻中隔矫正术及功能性鼻内窥镜术,术中出现鼻中隔黏膜一侧或对穿性破损,立即采用自体部分筛骨垂直板骨片移植封闭穿孔,复位破损黏膜,形成骨性支架,采用膨胀海绵填塞,术后鼻腔局部应用糖皮质激素,联合使用维生素AD滴剂湿润黏膜,促进周边黏膜向破损处生长。结果 28例患者均一次修复成功,随访1月,黏膜破损处骨质暴露区域基本被新生黏膜完全覆盖,所有患者随访3~6个月未见穿孔形成,均未出现手术并发症。结论鼻内镜下自体部分筛骨垂直板移植修补鼻中隔穿孔安全、简便、有效,是修补术中鼻中隔黏膜破损的最佳选择之一。
Objective To evaluate the clinical effects of Perpendicular plate of the ethmoid autotransplan- tation in repairing of nasal septal membrane injury during operation under endoscopy. Methods 28 patients with sinusitis and nasal polyps who need nasal septal surger, while the septal mucosa of the nasal septum was found on either side of the nasal septum, or in the case of wear. We adopt perpendicular plate of the ethmoid autograft immediately to close the perforation. After the operations, glucocorticoid and vitamin AD droups were used to promote nearby tissues grow towards the perforations. Results All cases were succes sfully repaired with the squamous epithelization in one month. Followed up from 3 to 6 months, no surgical complications happened. Conclusion Perpendicular plate of the ethmoid autotransplantation in repairing of nasal septal membrane injury during operation under endoscopy is safe, handy and effective.
