中性粒细胞/淋巴细胞比率与急性缺血性脑血管病的相关性 被引量:2

Correlation of neutrophil/lymphocyte ratio and acute ischemic cerebrovascular diseases

摘要 目的探讨中性粒细胞/淋巴细胞比率(NLR)与急性缺血性脑血管病的相关性。方法比较急性脑梗死(ACI)患者(ACI组,121例)、短暂性脑缺血发作(TIA)患者(TIA组,62例)和健康体检者(对照组,65例)NLR水平,分析NLR水平与ACI和TIA的相关性。发病后1个月随访,依据GOS将ACI组分为预后良好组(GOS评分4~5分)和预后不良组(GOS评分1~3分),比较两组患者NLR水平,分析NLR水平与ACI预后的相关性。结果 ACI组和TIA组NLR水平均高于对照组,且ACI组高于TIA组(P〈0.05);ACI预后不良组NLR水平高于预后良好组(P〈0.05);ACI组NLR水平与GOS评分呈负相关(r_s=-0.489,P〈0.01)。结论 NLR水平与急性缺血性脑血管病的临床分类及ACI预后密切相关,可用于ACI和TIA的鉴别诊断和ACI患者近期预后的评估。 Objective To investigate the correlation between neutrophil/lymphocyte ratio (NLR) and acute ischemic cerebrovascular diseases. Methods The NLR was compared among the patients with acute cerebral infarction(group ACI, 121 cases), transient ischemic attack(group TIA, 62 cases) and healthy people(group C, 65 cases). The correlation between NLR and ACI and TIA was analyzed. After followed up for one month, the patients were assigned into two groups of A1 (with poor outcome,CADS 1 to 3 points) and A2(with good outcome, GOS 4 to 5 points). The NLR was compared between groups of A1 and A2, and the correlation of NLR and prognosis of patients with ACI was analyzed. Results The NLR of group ACI and group TIA was significantly higher than that of group C(P〈0. 05),which was higher in group ACI than that in group TIA(P(0. 05). The NLR was higher in group A1 than that in group A2(P〈0. 05). The NLR in group ACI was negatively correlated with CADS score(r=-0. 489,P〈0. 01). Conclusion The NLR is closely related to the clinical classification of acute ischemic cerebrovascular diseases and the prognosis of ACI patients and may be taken as an indicator for the differential diagnosis of ACI and TIA and the evaluation of short- term prognosis of patients with ACI.
