胸骨正中切开及主动脉弓翻转行左肺癌淋巴结扩大清除术

EXTENDED MEDIASTINAL LYMPH NODE DISSECTION THROUGH MEDIAN STERNOTOMY AND THROUGH LIFT AORTIC ARCH FOR LEFT LUNG CANCER

我们对左侧肺癌,采取两种新的手术径路方法,即胸骨正中切开径路及主动脉弓翻转径路方法行纵隔淋巴结扩大清除术,前者手术径路能充分清除双侧上纵隔淋巴结。后者手术径路虽为后外侧开胸,但由于将主动脉弓及气管牵向一侧,亦很容易扩清气管右侧淋巴结。并对手术具体方法及特点进行讨论。

The two new procedures were applied for extended mediastinal lymph nodedissection through median sternotomy and through lift aortic arch in thetreatment left lung cancer. The dissection of bilateral superior mediastinallymph node with the former approach was sufficient. The aortic arch and itstrachea were drawn towards the right side with the later approach by lateralthoractomy. This procedure was easy for the dissection of right paratrachearlymph node.
