集束化护理干预对提高0～6个月婴儿纯母乳喂养率的影响

Effect of Cluster Nursing Intervention on Improving the Rate of Pure Breastfeeding of Infants in 0～6 Months

目的:探讨集束化护理干预对提高0~6个月婴儿纯母乳喂养率的影响。方法:选取2015年10~12月在某院建档并分娩的初产妇200例,随机分为实验组(100例)和对照组(100例),对照组采取常规的母乳喂养宣教方式进行,包括入院时、产后、出院宣教及出院后6个月随访;实验组采取集束化护理,分别在孕期、产前、产后<24h、24~48h及>48h进行宣教和指导、出院时宣教并在出院后1周、2周、1月、2月及6月进行回访和干预,对存在的问题及时进行处理,了解0~6个月婴儿纯母乳喂养率。结果:实验组产妇产后在<24h、24~48h及>48h纯母乳喂养效果明显高于对照组,两组对比差异有统计学意义(P<0.05);出院后1周、2周、1月、2月及6月婴儿纯母乳喂养率明显高于对照组,两组对比差异有统计学意义(P<0.05)。结论:集束化护理干预可有效进行母乳喂养相关知识宣教和指导,促进乳母乳汁分泌,建立母乳喂养的信心,对提高0~6个月婴儿纯母乳喂养率有积极的意义。

Objective：To investigate the effect of nursing intervention on breast feeding rate of increase of 0-6 months infants.Methods：200 cases of obstetric primipara treated in a hospital from October 2015 to De- cember 2015 were randomly divided into experimental group （100 cases） and control group （100 cases）.The control group received routine breast milk feeding education methods, including admission, postpartum, discharge education and 6 months follow-up after discharge.The experimental group adopted cluster nursing in- tervention, respectively during pregnancy, prenatal, postpartum - 24 hours, 24 - 48 hours and - 48 hours, the education and guidance were carried out. After discharge, the education was carried out in 1 weeks, 2 weeks, 1 month,2 months and 6 months, and the problems were dealt with in time. Then the rate of pure breastfeeding for 0-6 months was recorded.Results：The effect of pure breastfeeding in postpartum 24 hours, 24-48 hours and 48 hours in the experimental group was significantly higher than that of the control group, and the difference between the two groups was statistically significant （P〈0.05）;The rate of pure breastfeeding for infants in 1 weeks, 2 weeks,1 month,2 months and 6 months was significantly higher than that of the control group, and there was statistically significant difference between the two groups （P〈0.05）.Conclusion：Cluster nursing intervention can effectively teach and guide breastfeeding related knowledge, promote milk secretion,and establish breastfeeding confidence,which has a positive effect on raising the rate of pure breastfeeding for 0-6 months.
