头颈部训练在提高偏瘫患者平衡及下肢运动功能恢复中的作用

The role of head and neck training in improving the balance of hemiplegic patients and the recovery of lower limb motor function

目的探讨头颈部训练在促进偏瘫患者平衡及下肢运动功能恢复中的应用效果。方法根据入选标准选取我院在2015年7月~2016年9月收治的脑卒中偏瘫患者76例,按照随机数字表法分为试验组和对照组,每组各38例。对照组给予Bobath技术为主的常规康复训练,试验组在常规康复训练基础上增加头颈部训练,治疗4周后采用Fugl-Meyer下肢运动功能评定量表(FMA)、Bery平衡量表(BBS)、Tinetti步态评估量表(TGA)及10 m步行速度评价两组患者平衡及下肢运动功能恢复情况。结果两组患者平衡及运动功能与康复前比较有明显改善,差异有统计学意义(P<0.05);且试验组FMA量表、BBS量表、TGA量表及10 m步行速度评分与对照组比较优势明显,差异有统计学意义(P<0.05)。结论在常规康复训练基础上增加头颈部训练对促进偏瘫患者平衡及下肢运动功能恢复具有积极意义,能够提高患者生活质量,值得临床推广。

Objective To explore the effect of head and neck training in promoting balance of hemiplegia and recovery of lower limb motor function.Methods 76 patients with stroke hemiplegia admitted in our hospital from July 2015 to September 2016 were randomly divided into trial group and control group according to the random number table method.The rats in the control group were given routine rehabilitation training based on Bobath technique.The head neck training was trained on the basis of routine rehabilitation training in the trial group.After 4 weeks of treatment,the Fugl-Meyer lower limb motor function rating scale （FMA）,Bery balance scale （BBS）,Tinetti gait assessment scale （TGA） and 10 m walking speed were used to evaluate the balance of the two groups and the recovery of lower limb motor function.Results The balance and motor function of the two groups were signifcantly improved compared with those before recorery treatment （P〈0.05）.The FMA scale,BBS scale,TGA scale and 10 m walking speed score were compared with the control group,the difference was statistically significant（P〈0.05）.Conclusion Increasing head and neck training on the basis of routine rehabilitation training is of great significance to promote the balance of hemiplegic patients and the recovery of motor function of lower extremities.It can be more effective in improving the quality of life of patients and deserves clinical promotion.
