降钙素原、血小板检测对非感染性危重症患者预后评价

Evaluation of prognosis of patients with non-infectious critical illness by procalcitonin and platelet test

目的探讨血清降钙素原(PCT)及血小板计数(PLT)检测对非感染性危重症患者预后的评价作用。方法对我院ICU收治的非感染性危重症患者进行分析,入选患者按随访结果分为存活组及死亡组,并根据PCT检测结果不同将患者分为PCT低水平组48例(0.5μg/L≤PCT<2μg/L),PCT高水平组32例(2μg/L≤PCT≤5μg/L);依据PLT检测结果不同将患者分为PLT正常组49例(100~300)×109/L及PLT下降组31例,分别观察不同PCT水平及PLT患者APACHEⅡ评分及预后情况。结果存活组PCT水平明显低于死亡组(P<0.05),而PLT则明显高于死亡组(P<0.05);存活组患者APACHEⅡ评分明显低于死亡组(P<0.05);PCT低水平组APACHEⅡ评分及患者死亡率均明显低于PCT高水平组(P<0.05);PLT正常组APACHEⅡ评分及患者死亡率均明显低于PLT下降组(P<0.05)。结论非感染性危重症患者血清PCT异常升高和/或PLT异常下降,均提示患者预后不良。

Objective To investigate the prognostic value of serum procalcitonin（PCT） and platelet count（PLT）in patients with the noninfectious critically illness. Methods To analyze the ICU patients of our hospital were divided into survival group and death group, and were randomly divided into PCT group of 48 cases of low level（0.5 g/L≤ PCT 2 μg/L）, the high level of PCT group of 32 cases（2 μg/L ≤ PCT ≤ 5 μg/L） according to the results of PCT;according to the PLT detection results of different patients were divided into normal PLT group of 49 cases（100 ～ 300） × 10^9/L and PLT decreased group31 cases, observed APACHE Ⅱ score and prognosis of different levels of PCT and PLT in patients. Results The survival group＇ s PCT levels were significantly lower than the death group（P〈0.05）, while PLT was significantly higher than the death group（P〈0.05）; the APACHE score of survival group was significantly lower than the death group（P〈0.05）; APACHE Ⅱ score and mortality rate of low level of PCT group were significantly lower than the high level of PCT group（P〈0.05）; APACHE Ⅱ score and mortality rate of PLT normal group were significantly lower than those of group PLT decreased group（P〈0.05）. Conclusion The abnormal increase of serum PCT and/or abnormal PLT in patients with noninfectious critically illness indicate that the prognosis is bad.
