眼针治疗脑梗死后认知障碍的临床研究

Clinical Study of Eye Acupuncture in the Treatment of Cognitive Impairment after Cerebral Infarction

目的观察三组不同疗法对脑梗死后认知障碍患者的MMSE量表评分改善程度,评价眼针治疗脑梗死后认知障碍的临床疗效并探讨其作用机制。方法眼针治疗组选用双侧眼穴的肝区、肾区、脾区、心区。头穴对照组选取前神聪透悬厘穴;百会透曲鬓穴;承光穴;正营透承灵穴。眼针治疗组与头穴对照组均每日针刺1次,14 d为1疗程。药物对照组口服多奈哌齐片5 mg,1次/d,14 d为1疗程。1个疗程后通过MMSE量表评分观察不同治疗方法对脑梗死后认知障碍患者的认知功能改善程度。结果眼针治疗组MMSE量表升高分数明显大于对照组(P<0.01),说明眼针治疗效果优于其他对照组。结论眼针疗法能够显著改善脑梗死后认知障碍患者的MMSE量表评分。其作用机制可能与增加乙酰胆碱含量,改善颅脑供血,促进脑细胞的恢复有关。

Objective To observe the effect of three groups of different therapies on the improvement of MMSE scale score in patients with cognitive impairment after cerebral infarction, and to evaluate the clinical efficacy of eye acupuncture in the treatment of cognitive impairment after cerebral infarction and to explore its mechanism. Methods The liver, renal, splenic and heart areas of bilateral eye acupoints were selected in the eye acupuncture treatment group. Head acupoints control group before Shencong-Xuanli acupoint; Baihui Qubin scalp; Chengguang acupoint; Zhengying-Chengling acupoint. The eye acupuncture treatment group and the scalp acupuncture control group were treated with acupuncture 1 times a day, 14 days for 1 courses. The drug control group took donepezil tablets 5 mg, 1 times a day, 14 days for 1 courses. After 1 courses of treatment, the cognitive function of patients with cognitive impairment after cerebral infarction was observed by MMSE scale. Results The MMSE scores of the eye acupuncture group were significantly higher than those of the control group（P〈0.01）, indicating that the eye acupuncture treatment was better than the other control group. Conclusion Eye acupuncture therapy can significantly improve the MMSE scale score in patients with cognitive impairment after cerebral infarction. Its mechanism may be related to the increase of acetylcholine content, the improvement of cerebral blood supply, and the promotion of brain cell recovery.
