音乐疗法对发声困难儿童神经康复护理过程中依从性的影响

Effects of music therapy on compliance in neurological rehabilitation nursing of children with vocal difficulty

目的研究音乐疗法对发声困难儿童康复护理过程中依从性的影响。方法共纳入28例发声困难儿童,随机分为研究组与对照组各14例,研究组在接受标准化治疗、护理的同时通过音乐疗法进行干预,对照组仅行标准化护理。记录分析2组平均练习的频率、发声质量及对治疗的感受总结。结果 2组智力发展水平评估均为中下水平,组间比较差异无统计学意义(P>0.05),排除严重神经系统器质性病变;研究组平均练习频率显著高于对照组(2.88±1.22vs 1.70±1.11),差异有统计学意义(P=0.027);各年龄组音乐治疗时平均练习频率显著高于标准化护理患者;干预护理前2组的发声障碍水平基本一致,护理后研究组发声障碍严重程度显著降低,CAPE-V评分与pVHI总分干预前后比较差异有统计学意义(P<0.05)。结论音乐疗法能够显著改善语言障碍患儿康复护理及练习的依从性,提高平均练习频率,改善其发音质量,值得临床应用。

Objective To observe the effect of music therapy on compliance in rehabilitation nursing for children with dysphonia. Methods 28 subjects were randomly divided into study group and control group. The patients were treated with standardized and music therapy in the study group. The patients in the control group only accepted standardized treatment care. The average exercise frequency, sound quality and summary of the treatment feelings were recorded. Results The level of mental intelligence in the two groups was in the middle and lower levels,and there was no significant difference between the two groups. The average frequency of exercise in the study group was significantly higher than that in the control group （P = 0, 027）. Subjects in any age group treated with music therapy showed significantly higher average daily practice frequency than that treated with standard nursing therapy. The first two groups in the study group had similar level of vocal difficulty before the intervention. After the music therapy, the study group showed significantly lower vocal difficulty. There was significant difference between the CAPE-V score and the pVHI score before and after the music treatment in the study group. Conclusion Music therapy can improve patient compliance, increase the frequency of average exercise and improve the quality of pronunciation in children with vocal difficulty.
