ADC值在鉴别间变型与非间变型室管膜瘤中的应用

摘要 目的:探讨扩散加权成像和表观系数扩散(ADC)值在鉴别间变型管膜瘤与非间变型室管膜瘤中的价值。方法:回顾性分析我院经手术病理证实的52例室管膜瘤患者的病例资料,其中非间变型室管膜瘤27例,间变型室管膜瘤25例。测量肿瘤实质部分的最小、平均及最大ADC值,并采用ROC曲线分析其诊断效能。结果:间变型室管膜瘤的最小ADC值[(0.808±0.190)×10^(-3)mm^2/s]低于非间变型室管膜瘤[(1.093±0.365)×10^(-3)mm^2/s],差异有统计学意义(P<0.05);鉴别两种肿瘤的最佳最小ADC值为0.887×10^(-3)mm^2/s,诊断敏感度为78.5%、特异度为73.2%。两种类型肿瘤的平均ADC值和最大ADC值的差异无统计学意义(P>0.05)。结论:最小ADC值有助于间变型室管膜瘤与非间变型室管膜瘤的鉴别诊断。 Objective:To investigate the value of magnetic resonance diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) in the differential diagnosis of anaplastic ependymomas and nonanaplastic ependymomas. Methods:52 patients with ependymomas were recruited,including 27 cases with nonanaplastic ependymomas and 25 cases with anaplastic ependymomas. The ADCmin, ADCmean and ADCmax of parenchyma of different tumors were measured and their receiver operating characteristic (ROC) curves were delineated. The optimum ADC value for differential diagnosis of anaplastic ependymomas and nonanaplastic ependymomas was analyzed and determined. Results.. There was no statistically significant difference between the ADCmean and ADCmax values of the two types of ependymomas, while the ADCmin was statistically significant. The mean ADCmin of anaplastic ependymomas and nonanaplastic ependymomas was (0. 808 ±0. 190) × 10-3 and ( 1. 093 ± 0. 365) ×10 3 mm2/s, respectively. The optimum ADCmin to distinguish anaplastic ependy- momas from nonanaplastic ependymomas was 0. 887 ×10-3 mm2/s with sensitivity of 78.5 % and specificity of 73.2 %. Con- clusion: ADCmin is very helpful for differential diagnosis of anaplastic ependymomas and nonanaplastic ependymomas
