2型糖尿病及抑郁症合并2型糖尿病患者事件相关电位P_(300)的比较

COMPARISON OF EVENT-RELATED POTENTIAL VISUAL P_(300) BETWEEN PATIENTS WITH TYPE 2 DIABETES MELLITUS AND DEPRESSION COMPLICATED WITH TYPE 2 DIABETES MELLITUS

目的:探讨事件相关电位(ERP)P_(300)在2型糖尿病(2DM)及抑郁症合并2型糖尿病患者中的特点。方法:符合WHO1999年制定的DM诊断标准以及CCMD-3制定的抑郁症诊断标准,收集到2DM患者30例(2DM组)和抑郁症合并糖尿病患者28例(2D/DM组)以及健康对照患者30例(NC组)。使用中国仪器以及"视觉靶-非靶刺激序列"为诱发事件,完成视觉P_(300)检测。结果:⑴2DM组与D/DM组变化表现为同一趋势,即潜伏期延迟,波幅降低。⑵2DM组、D/DM组及NC组在靶潜伏期P_z脑区N_2和P_3,靶波幅P_3及非靶波幅P_2差异均有统计学意义(P<0.05或0.01)。⑶与NC组比较,D/DM组视觉P_(300)靶潜伏期N_2和P_3延迟(P<0.01),靶波幅P_3降低(P<0.01)。与NC组比较,2DM组也见相同结果。⑷在非靶刺激中,与NC组比较,D/DM组和2DM组波幅均下降。D/DM组与2DM组比较,前者波幅又低于后者(P<0.01)。结论:D/DM组比2DM组视觉P_(300)变化更大,视觉P_(300)有可能作为2型糖尿病和抑郁症合并糖尿病患者辅助诊断的一个脑电生理学标志。

Objective： To explore the characteristics of event related potential （ERP） P30o in patients with type 2 diabetes mellitus （2DM） and depression complicated with type 2 diabetes mellitus. Methods： Thirty patients with 2DM （2DM group） and 28 patients with depression complicated with diabetes mellitus （2D/DM group） and 30 healthy controls （NC group） were enrolled the study, who reached the DM diagnostic criteria set in 1999 by WHO and CCMD-3 depression diagnostic criteria. The ERP P300 was elicited using chinese machine and an oddball task .Results： （1） Visual P300 were found delayed ERP latency and reduced amplitude in 2DM and D/ DM group. （2）There were significant differences in target latency elicited at Pz points in both N2 and P3 among the three group. And so were the target amplitude of P3 and non-target amplitude of P2 （P〈0.05 or 0.01）. （3） Compared with NC group, both N2 and P3 target latency of visual P300 were found delayed, and the target amplitude of P3 was decreased in 2DM group and D/DM group （P〈0.01）. （4） Compared with NC group, the amplitude were decreased in non-target stimulation in both D/DM and 2DM group, and more lower in D/DM group （P〈0.01）. Conclusion： The changes of visual P300 more significantly in D/DM group than those in 2DM group, and visual P3oo may be a marker of electrophysiological diagnosis in patients with depression complicated with type 2 diabetes mellitus and type 2 diabetes mellitus.
