急性睾丸扭转30例临床诊治体会及相关文献复习

Diagnosis and treatment of 30 cases of acute testicular torsion and related literature review

目的:提高急性睾丸扭转的认识、早期诊断及治疗水平。方法:回顾性分析我院2011年6月至2016年10月间经超声及手术探查确诊的30例急性睾丸扭转病例资料。结果:30例睾丸扭转病例中,1例行手法复位成功,其余29例行手术探查,其中11例行患侧睾丸扭转复位固定术加健侧睾丸固定术,18例行患侧睾丸切除术加健侧睾丸固定术。结论:急性睾丸扭转是导致男性、特别是青春期少年睾丸缺失的主要疾病,超声多普勒检查可较准确的发现睾丸血流减少或消失,及早手术探查及治疗可较早恢复睾丸血流信号,降低睾丸损失。

Objectives： To improve the knowledge,early diagnosis and treatment of acute testicular torsion.Methods： Retrospective analysis of 30 cases of acute testicular torsion diagnosed by ultrasound and surgery in our hospital from June 2011 to October 2016 was conducted. Results： Among the 30 cases of testicular torsion,there was a case that underwent manipulative reduction successfully. The other 29 cases underwent laparotomy,including11 cases of testicular torsion fixation and contralateral orchiopexy,and 18 cases of ipsilateral orchiectomy and contralateral orchiopexy. Conclusion： Acute testicular torsion is especially the main disease causing adolescents absence of testis. Doppler ultrasound can accurately detect testicular blood flow decreased or disappeared. Early surgical exploration and treatment can restore the testicular blood flow early to decrease the testicular damage.
