不同手术入路在腰椎间盘突出症治疗中的临床疗效对比研究

Effect of treatment with different surgical approach in lumbar disc protrusion

目的观察不同手术入路治疗腰椎间盘突出症的临床效果。方法选择2015年4月~2016年7月在我院接受治疗的腰椎间盘突出症患者90例,随机分为对照组和观察组各45例。对照组采用腰椎后路正中切口入路手术治疗,观察组采用多裂肌间隙入路手术治疗,观察比较两组患者的手术时间、术中出血量、术后疼痛(VAS)评分,并使用改良Macnab对两组患者的治疗效果进行评价。结果观察组的手术时间、术中出血量均少于对照组,VAS评分低于对照组,治疗效果(93.33%)明显高于对照组(75.56%),差异均有统计学意义(P<0.05)。结论多裂肌间隙入路手术治疗腰椎间盘突出症的效果理想,值得临床推广使用。

Objective To observe the clinical effects of treatment with different operative approach in lumbar disc protrusion. Methods 90 cases of lumbar disc protrusion from April 2015 to July 2016 in our hospital,were randomly divided into control group and observation group(45 cases each).Used of the surgical treatment of lumbar posterior midline incision approach in control group,the surgical treatment of the road from multifidus muscular clearance into for the observation group. Results Observation group of the operation time,intraoperative blood loss,and postoperative pain(VAS)were less than the control group.The 93.33% of treatment effect is significantly higher than the 75.56%in control group(P<0.05). Conclusion The effect is well in the surgical the road from multifidus muscular clearance into for treatment of lumbar disc protrusion.
