奥曲肽联合埃索美拉唑治疗非静脉曲张急性上消化道出血的疗效分析

Curative effect of Octreotide combined with Esomeprazole for treatment of acute no-varicosity upper gastrointestinal bleeding

目的观察醋酸奥曲肽联合埃索美拉唑治疗急性非静脉曲张上消化道出血的疗效。方法选择2015年6月~2017年1月我科住院治疗的重度非静脉曲张性上消化道出血患者224例,随机分为对照组76例,试验A组53例,试验B组46例,试验C组49例。对照组予以埃索美拉唑治疗,试验A组在对照组治疗的基础上加用奥曲肽0.1mg皮下注射后继以奥曲肽25μg/h持续静脉泵注,试验B组在对照组治疗的基础上加用奥曲肽0.1mg皮下注射后继以奥曲肽以50μg/h持续静脉泵注,试验C组在对照组治疗的基础上加用奥曲肽0.1mg皮下后继以奥曲肽75μg/h持续静脉泵注,观察比较各组的临床治疗效果。结果对照组总有效率为76.32%,试验A组、试验B组、试验C组总有效率分别为90.57%、91.30%、91.83%,对照组与试验A组、试验B组、试验C组之间两两比较,差异具有统计学意义(P<0.05);试验A组、试验B组、试验C组之间两两比较,差异无统计学意义(P>0.05)。结论在治疗非静脉曲张上消化道出血时,奥曲肽泵入联合埃索美拉唑较单用埃索美拉唑的治疗效果显著,值得临床推广应用。

Objective To observe the curative effect of the Octreotide combined with Esomeprazole for treatment of acute no-varicosity upper gastrointestinal bleeding. Methods 224 cases of upper non varicose veins gastrointestinal hemorrhage,from June 2015 to January 2015 in our hospital were randomly divided into control group of 76 cases,experimental group A of 53 cases,46 cases of experimental group B,and group C test in 49 cases.Control group was treatment with Esomeprazole,the experimental group A was on the basis of control group therapy plus with Octreotide 0.1 mg subcutaneous injection subsequent to the Octreotide 25μg/h continuous intravenous injection pump,the experimental group B basis on the control group therapy combined with Octreotide 0.1 mg for subcutaneous injection,than with Octreotide 25μg/h continuous iv drip by injection pump,C group was basis on the control group treated with Octreotide 0.1 mg subcutaneous injection,than given Octreotide 25μg/h continuous iv drip by injection pump. Results The control effective rate was 76.32%,experiment group A was 90.57%,and group B and C group effective rate was 91.30%and 91.83%.Comparison the control group between to the experiment group A,group B,and group C,was statistically significant difference(P<0.05). Conclusion The therapy effect of Octreotide combined with Esomeprazole in the treatment of non varicose veins upper gastrointestinal bleeding was more significantly than only treatment with Esomeprazole.
