经阴道超声造影在宫颈癌诊断中的应用评价

Evaluation of application effect of transvaginal ultrasound angiography in diagnosis of cervical cancer

目的探讨经阴道超声造影在宫颈癌诊断中的应用价值。方法选取该院2015年6月-2016年12月间收治的疑似宫颈癌患者200例作为研究对象,分别对其实施常规超声、经阴道超声造影检查,以病理诊断结果为依据,计算比较两种方法判断宫颈癌病理分期、浸润程度的符合率。同时,分析不同类型宫颈病变在经阴道超声造影检查时的增强参数变化情况。结果200例疑似宫颈癌患者经术后病理诊断,共确诊宫颈癌172例,宫颈上皮内瘤变23例,正常5例。常规超声和经阴道超声造影诊断宫颈癌病理分期符合率分别为61.6%和81.4%,诊断宫颈癌浸润深度符合率分别为75.0%和91.3%,经阴道超声造影均显著高于常规超声,两组比较差异有统计学意义(均P<0.05)。同时,宫颈癌患者经阴道超声造影检查时的增强强度显著高于宫颈上皮内瘤变患者和正常宫颈受检者,而增强开始时间、峰值时间则均呈相反规律,3组比较差异有统计学意义(均P<0.05)。结论经阴道超声造影检查可以为宫颈癌的病理分期、浸润深度判断提供可靠依据,对于宫颈癌的诊断、分期、治疗、预后评价都有十分重要的意义。

Objective To explore the application value of transvaginal ultrasound angiography in diagnosis of cervical cancer.Methods A total of 200 patients with suspected cervical cancer treated in Maternal and Child Health Care Hospital of Luanping County from June 2015 to December 2016 were selected,conventional ultrasonography and transvaginal ultrasound angiography were performed,respectively.The coincidence rates of pathological stages and the degrees of invasion of cervical cancer patients detected by the two methods were calculated and compared based on the results of pathological diagnosis.The changes of enhancement parameters of transvaginal ultrasound angiography for different types of cervical lesions were analyzed.Results After postoperative pathological diagnosis,172 patietns were diagnosed as cervical cancer,23 patietns were diagnosed as cervical intraepithelial neoplasia （CIN）,and 5 cases were found with normal cervix.The coincidence rates of pathological stages of cervical cancer patients detected by conventional ultrasonography and transvaginal ultrasound angiography were 61.6% and 81.4%,respectively.The coincidence rates of degrees of invasion of cervical cancer patients detected by conventional ultrasonography and transvaginal ultrasound angiography were 75.0% and 91.3%,respectively.The coincidence rates of pathological stages and the degrees of invasion of cervical cancer patients detected by transvaginal ultrasound angiography were statistically significantly higher than those by conventional ultrasonography （P〈0.05）.The enhancement degree of transvaginal ultrasound angiography in cervical cancer patients was statistically significantly higher than those in CIN patients and normal women,while the start time and peak time of enhancement in cervical cancer patients was statistically significantly lower than those in CIN patients and normal women （P〈0.05）.Conclusion Transvaginal ultrasound angiography can provide a reliable basis for determination of pathological stages and degree of invasion of cervical cancer,which is very important in diagnosis,staging,treatment,and prognosis evaluation of cervical cancer.
