深圳16887例不同胎龄单胎新生儿艾里斯曼指数研究

Elisma index of singleton neonates in different gestational age on Shenzhen city

目的:制定不同胎龄新生儿艾里斯曼指数(Elisma index,EI)百分位曲线,为评价新生儿出生时身体匀称性提供参考数据。方法:采用横断面时间段整群抽样实况调查方法于2013年4月至2015年9月在深圳两家医院完成16 887例27~42周胎龄单胎新生儿体质量、身长、顶臀长、头围、胸围现场测量,用以制定不同胎龄新生儿EI百分位曲线。曲线拟合应用Lambda Mu Sigma(LMS)方法。结果:得到2013—2015年深圳16 887例27~42周胎龄单胎新生儿(男、女、合计)三分组EI均数和3rd^97th百分位曲线。EI在27周胎龄最低,随着胎龄增加EI不断增大,42周胎龄EI最大。男性EI百分位50th曲线形态和变化趋势与女性50th曲线一致。27~37、41~42周胎龄,男性EI的50th百分位曲线与女性曲线几乎重叠,男比女EI略减少0.01~0.07,但差异无统计学意义P>0.05。38~40周胎龄,男性EI的50th曲线值比女性曲线减少了0.11~0.13,差异有统计学意义(t=2.72~4.00,P<0.01)。结论:不同胎龄新生儿胸廓发育度伴随胎龄增加不断提高,42周胎龄胸廓发育度最佳。38~40周胎龄女性新生儿出生时胸廓发育好于男性。

Objective：To create Elisma index（EI） percentile curves of singleton neonates in different gestational age（GA）,so as to provide a reference data for evaluating enrichment degree of neonates at birth.Methods：A total of 16 887（9 418 male,7 469 female） live singleton neonates at 27-42 weeks of GA were measured at birth by site survey method using cluster samples by a cross sectional time in two Hospital of Shenzhen City from April 2013 to September 2015 in this study,to create EI percentile curves of singleton neonates in different GA.Percentile curves were smoothed by Cole TJ.Lambda Mu Sigma（LMS） method.Results：The mean EI and 3 rd-97 th percentile curves of singleton neonates（male,female,and unisex three grouping） in 27-42 weeks of GA were created in Shenzhen city,China in 2015.The EI lowest values were at 27 weeks of GA,and the highest values appear at 42 weeks of GA.The shape and change trend for EI 50 th curves of male were basically identical with that of female.The EI 50 th percentile curves of male with female were almost overlap in 27-37 and 41-42 weeks of GA,the curve values of male were decreased 0.01-0.07 than female（P〉0.05,with no statistical significance）.The50 th curve values of male were decreased 0.11-0.13 than female（t=2.72-4.00,P〈0.01-P〈0.001,with a statistical significance） in 38-40 weeks of GA.Conclusion：The chest fullness and enrichment degree of neonates in different GA were increased along with GA increasing.The EI of neonates reached peak in 42 weeks of GA.The chest fullness degree of female was higher than that of male in 38-40 weeks of GA.
