比较局部麻醉和脊椎麻醉在膝关节镜手术中的效果及安全性

Effect and Safety of Local Anesthesia and Spinal Anesthesia in Knee Arthroscopic Surgery

目的对局部麻醉和脊椎麻醉在膝关节镜手术中的应用效果及安全性进行分析和比较。方法在我院2015年8月~2017年1月期间收治的膝关节镜手术患者中选取100例作为研究对象,根据随机双盲原则将患者平均分为观察组和对照组两组,每组50例,观察组患者接受局部麻醉,对照组患者接受脊椎麻醉,比较两组患者手术效果及手术安全性。结果观察组患者术后不同时点的VAS评分均显著低于对照组患者(P<0.05);此外,相较于对照组患者,观察组患者麻醉总满意率更高、术后不良反应总发生率更低(94.00% vs 72.00%、4.00% vs 24.00%,P<0.05)。结论在膝关节镜手术中,局部麻醉的应用效果及安全性优于脊椎麻醉,可显著降低患者术后疼痛感、减少不良反应,推荐使用。

Obje c t ive To investigate the effect and safety o f local anesthesia and spinal anesthesia in knee arthroscopic surgery. Methods A total of 100 patients underwent knee arthroplasty from August 2015 to January 2017 were randomized in-to the observation group and control group using random double blind, 50 cases in each group. T h e control group was treated with spinal anesthesia. The observation group received local anesthesia. The clinical effect and safety were compared between the two groups. Results The VAS scores of the observation group were significantly lower than those of the control group at different times after surgery （P〈0 . 05）. T h e rates o f satisfaction and adverse reactions in the observation group were better than those in the control group （94. 00% vs 72. 00% , 4. 00% vs 24. 0 0% , P〈0 . 05）. Conclusion Local anesthesia has better effect and safety than spinal anesthesia in the knee arthroplasty, which can reduce the postoperative pain and adverse reac-tions, worthy of clinical application.
