久泻宁颗粒联合白头翁汤治疗慢性非特异性溃疡性结肠炎疗效观察及对血清几丁质酶-3样蛋白-1含量的影响

Effects observation of Jiuxiening granule combined with Pulasatilla decoction on ulcerative colitis and the content of serum YKL-40

目的观察久泻宁颗粒联合白头翁汤治疗慢性非特异性溃疡性结肠炎(CUC)临床疗效,以及对血清几丁质酶-3样蛋白-1(Chtinase-3-like-1 protein,YKL-40)含量的影响。方法将60例CUC患者随机分为2组,治疗组30例予久泻宁颗粒联合白头翁汤治疗,对照组30例予美沙拉嗪颗粒剂治疗。2组均治疗8周后统计综合疗效、中医证候疗效及肠镜下黏膜疗效,并观察2组治疗前后症状积分、疾病活动指数评分(DAI)及YKL-40情况。结果临床综合疗效:治疗组总有效率90.0%,对照组总有效率76.7%,2组比较差异无统计学意义(P>0.05)。中医证候疗效:治疗组总有效率90.0%,对照组总有效率76.7%,2组比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。肠镜下黏膜疗效:治疗组总有效率83.33%,对照组总有效率73.33%,2组比较差异无统计学意义(P>0.05)。2组治疗后症状总积分与本组治疗前比较差异均有统计学意义(P<0.05),但2组治疗后症状总积分组间比较差异无统计学意义(P>0.05)。2组治疗后主要症状积分均降低(P<0.05),且治疗组治疗后腹泻、里急后重症状积分降低优于对照组(P<0.05)。2组治疗后DAI评分与本组治疗前比较差异均有统计学意义(P<0.05),但2组治疗后组间比较差异无统计学意义(P>0.05)。2组治疗后血清YKL-40水平与本组治疗前比较差异均有统计学意义(P<0.05),但2组治疗后组间比较差异无统计学意义(P>0.05)。结论久泻宁颗粒联合白头翁汤治疗CUC疗效确切,并可改善患者的临床症状及中医症状,同时降低血清YKL-40水平。

Objective To observe the clinical effects of Jiuxiening gruanule combined with Pulasatilla decoction on chronic ulcerative colitis( CUC) and the content of serum chtinase-3-like-1 protein( YKL-40). Methods 60 CUC patients were randomly divided into two groups. 30 cases in the treatment group were treated by Jiuxiening gruanule combined with Pulasatilla decoction,and 30 cases in the control group were treated by mealazine gruanule.The comprehensive efficacy,TCM syndrome efficacy and the colonoscopic mucosal efficacy after treatment 8 weeks were evaluated in two groups. And the symptom scores,disease activity index( DAI) scores and YKL-40 before and after treatment were observed in two groups. Results There was no statistical difference on the comprehensive efficacy between the treatment group( 90. 0%) and the control group( 76. 7%,P > 0. 05). There were statistical differences on the TCM syndrome efficacy between the treatment group( 90. 0%) and the control group( 76. 7%,P < 0. 05),and the effects in the treatment group were superior to that in the control group( P < 0. 05). There was no statistical difference on the colonoscopic mucosal efficacy between the treatment group( 83. 33%) and the control group( 73. 33%,P >0. 05). There were statistical differences on the total symptom scores between before and after treatment in two groups( P < 0. 05). There was no statistical difference on the total symptom scores after treatment between the two groups( P >0. 05). The main symptom scores decreased after treatment in two groups( P < 0. 05),and the reduction of scores of diarrhea and tenesmus after treatment in the treatment group was superior to that in the control group( P < 0. 05).There were statistical differences on the DAI scores between before and after treatment in two groups( P < 0. 05).There was no statistical difference on the DAI scores after treatment between the two groups( P > 0. 05). There were statistical differences on the serum YKL-40 between before and after treatment in two groups( P < 0. 05). There was no statistical difference on the serum YKL-40 after treatment between the two groups( P > 0. 05). Conclusion Jiuxiening gruanule combined with Pulasatilla decoction has exact effects on the the UC,and it can improve the clinical symptoms and TCM symptoms,and reduce the level of serum YKL-40.
