背部特定组穴拔罐治疗小儿感染后咳嗽临床观察及对免疫功能的影响

Clinical observation of cupping in back specific acupoints on children cough after infection and its effects on the immune function

目的观察背部特定组穴拔罐治疗小儿感染后咳嗽的临床疗效及对免疫功能的影响。方法将100例感染后咳嗽患儿随机分为2组,对照组50例予盐酸氨溴索口服液治疗,治疗组50例采用背部特定组穴拔罐治疗。2组均治疗12 d后统计疗效,比较2组治疗前后中医证候评分变化情况,肺功能[50%肺活量最大呼气流量(V50)、第1 s用力呼气容积(FEV1)占用力肺活量(FVC)比值(FEV1/FVC)]、免疫学指标[血清免疫球蛋白G(IgG)、IgA、可溶性白细胞介素-2受体(SIL-2R)]改善情况,并比较2组患儿咳嗽消失时间;统计治疗痊愈患儿6个月后咳嗽发作次数。结果治疗组总有效率96.0%,对照组总有效率72.0%,治疗组优于对照组(P<0.05)。2组治疗后咳嗽、咯痰评分较治疗前均降低(P<0.01);治疗组治疗后咳嗽、咯痰评分低于对照组(P<0.05)。2组治疗后FEV1/FVC、V50较治疗前均升高(P<0.05);治疗组治疗后FEV1/FVC、V50均高于对照组(P<0.05)。2组治疗后IgG、IgA均升高,SIL-2R降低(P<0.05);治疗组治疗后Ig G、IgA高于对照组,SIL-2R低于对照组(P<0.05)。治疗组痊愈时间短于对照组(P<0.05)。治疗组痊愈患儿6个月内呼吸系统疾病发作次数少于对照组(P<0.05)。结论背部特定组穴拔罐治疗小儿感染后咳嗽疗效肯定,可明显改善肺功能,升高IgG、IgA,降低SIL-2R,提高机体免疫力,缩短疗程,并能有效预防呼吸系统疾病的发生,且安全无毒副作用,体感舒适易于接受等特点,值得进一步推广。

Objective To observe the clinical effects of cupping in back specific acupoints on children cough after infection and its effects on the immune function. Methods 100 children with cough after infection were randomly divided into two groups. The control group( n = 50) were treated by ambroxol hydrochloride oral solution. The treatment group( n = 50) were treated by cupping in back specific acupoints. The effects were evaluated after treatment 12 d in two groups. The changes of traditional Chinese medicine syndrome score,the pulmonary function including 50% maximal expiratory flow of vital capacity( V50),the ratio of the forced expiratory volume in 1 second( FEV1)/forced vital capacity( FVC,FEV1/FVC),the immunological indexes including serum immunoglobulin G( Ig G),immunoglobulin A( Ig A) and soluble interleukin-2 receptor( SIL-2 R),and the disappear time of cough before and after treatment were compared in two groups,and the cough episodes of children cured were evaluated 6 months after treatment. Results The total effective rate in treatment group( 96. 0%) was higher than that in control group( 72. 0%,P < 0. 05). The scores of cough and expectoration after treatment were decreased in two groups( P <0. 01),and which in treatment group were lower than those in control group( P < 0. 05). The FEV1/FVC and V50 after treatment were increased in two groups( P < 0. 05),and which in treatment group were higher than those in control group( P < 0. 05). The Ig G and Ig A after treatment were increased in two groups,and the SIL-2 R reduced( P < 0. 05),the Ig G and Ig A in treatment group were higher than those in control group( P < 0. 05),and the SIL-2 R was lower( P < 0. 05). The healing time of the treatment group was shorter than that in the control group( P < 0. 05). The episodes number of respiratory diseases in treatment group was less than that in the control group within 6 months( P < 0. 05). Conclusion Cupping in back specific acupoints has exact effects on children cough after infection,can obvious improve the pulmonary function,increase Ig G and Ig A,reduce SIL-2 R,enhance the body immunity,shorten the treatment course,and can effectively prevent the occurrence of respiratory diseases,safe and non-toxic side effects,comfort and easy to accept and so on,worthy of further promotion.
