高b值磁共振弥散加权成像诊断前列腺癌的临床价值

Diagnostic value of high b-value diffusion-weighted imaging in prostate cancer

目的探讨高b值的弥散加权成像(DWI)对前列腺癌的诊断价值。方法收集2014年5月~2017年1月在东南大学附属江阴医院行MRI扫描的67例疑诊前列腺癌患者,根据病理结果比较DWI(b=800,1500,2 000s/mm^2)诊断前列腺癌的敏感度、特异度、诊断符合率及受试者工作特征曲线下面积差异。结果高b值DWI诊断前列腺癌的诊断符合率、敏感度、特异度及AUC面积均高于常规b值DWI,差异有统计学意义,DWI(b=1 500s/mm^2)诊断前列腺癌敏感度高于DWI(b=2 000s/mm^2)。结论高b值DWI在前列腺癌诊断中具有较高价值,尤其1 500s/mm^2是诊断相对更佳的选择。

Objective To explore the value of high b-value DWI in diagnosis of prostate cancer. Methods Sixty-seven patients with clinical suspicion of prostate cancer were scanned with MRI,all cases were pathological diagnosis as the gold standard,and the areas under the curves（ AUCs）,sensitivity,specificity and predictive value differences between DWI（ b = 800,1 500,2000 s/mm^2） were analyzed. Results The areas under the curves（ AUCs）,sensitivity,specificity and predictive value for high b-value DWI were significantly higher than conventional DWI,and the sensitivity for DWI（ b = 1 500 s/mm2） was high than DWI（ b = 2 000 s/mm^2）. Conclusion The high b-value DWI is a valuable MRI modality in the diagnosis of prostate cancer.Especially,1 500 s/mm^2 is a better option.
