无创呼吸机不同通气模式治疗慢阻肺合并呼吸衰竭的临床疗效分析

Clinical efficacy analysis of noninvasive ventilation with different ventilation modes in the treatment of chronic obstructive pulmonary disease complicated with respiratory failure

目的分析无创呼吸机不同通气模式治疗慢性阻塞性肺疾病(慢阻肺)合并呼吸衰竭的临床疗效。方法 50例慢阻肺合并呼吸衰竭患者,采用等距随机抽样法分为对照组及观察组,各25例。两组患者均接受无创呼吸机治疗,对照组采用ST通气模式;观察组采用平均容积保证压力支持(AVAPS)通气模式。对两组临床疗效进行比较分析。结果治疗前两组经皮血氧饱和度、动脉血气指标比较差异无统计学有意义(P>0.05)。治疗后观察组经皮血氧饱和度、p H值、动脉血氧分压、动脉血二氧化碳分压均优于对照组,差异有统计学意义(t=14.943、5.466、17.765、19.838,P<0.05)。结论无创呼吸机不同通气模式治疗慢阻肺合并呼吸衰竭可取得理想疗效,但AVAPS通气模式效果更佳。

Objective To analyze the clinical efficacy of noninvasive ventilation with different ventilation modes in the treatment of chronic obstructive pulmonary disease complicated with respiratory failure. Methods A total of 50 patients with chronic obstructive pulmonary disease complicated with respiratory failure were divided by equidistant random sampling method into control group and observation group, with 25 cases in each group. Both groups were treated with noninvasive ventilation. The control group received ST ventilation mode, and the observation group received ventilation mode of average volume assured pressure suppont（AVAPS）. The clinical efficacy in two groups was compared and analyzed. Results Before treatment, both groups had no statistically significant difference in percutaneous oxygen saturation, arterial blood gas（P〉0.05）. After treatment, the observation group had better percutaneous oxygen saturation, p H value, arterial oxygen partial pressure, arterial blood carbon dioxide partial pressure than the control group, and their difference was statistically significant（t=14.943, 5.466, 17.765, 19.838, P〈0.05）. Conclusion Noninvasive ventilation with different ventilation modes shows ideal efficacy in treating chronic obstructive pulmonary disease complicated with respiratory failure, but AVAPS ventilation mode has better effect.
