血浆BNP水平在慢性心衰诊断及预后评估中的应用价值 被引量:17

摘要 目的对血浆脑钠肽(BNP)水平在慢性心力衰竭(心衰)诊断及预后评估中的应用价值进行探讨。方法选取心血管疾病患者(心功能正常)50例作为对照组,另选取同期收治的慢性心衰患者50例作为观察组。检测比较两组患者入院时血浆BNP、左室舒张末期内径(LVEDD)、左室射血分数(LVEF)水平;记录观察组患者治疗后的BNP及LVEF水平,并与治疗前作比较。结果观察组各心功能分级患者的BNP、LVEDD水平均高于对照组,LVEF水平均低于对照组,差异具有统计学意义(P<0.05);患者的BNP、LVEDD水平随着心功能分级的升高而升高,LVEF水平随着心功能分级的升高而降低。观察组患者治疗前血浆BNP水平为(273.46±186.58)pg/ml,LVEF为(35.18±4.73)%;治疗后血浆BNP水平为(95.20±56.92)pg/ml,LVEF为(43.18±7.35)%。观察组患者治疗后血浆BNP水平低于治疗前、LVEF高于治疗前,差异有统计学意义(P<0.05)。结论血浆BNP水平在慢性心衰的诊断及预后评估中有着很高的应用价值。 Objective To discuss the application value of plasma brain natriuretic peptide (BNP) level in the diagnosis and prognostic evaluation of chronic heart failure. Methods There were 50 patients with cardiovascular disease (normal cardiac function) as control group, and concurrent 50 patients with chronic heart failure as observation group. The levels of plasma BNP, left ventricular end diastolic diameter (LVEDD) and left ventricular ejection fraction (LVEF) were compared between the two groups. The level of BNP and LVEF after treatment in the observation group was recorded and compared with that before treatment. Results The observation group had higher BNP and LVEDD of patients with different cardiac function than the control group, and lower LYEF level than the control group. Their difference was statistically significant (P〈0.05). The level of BNP and LVEDD increased with the increase of cardiac function classification, and the level of LVEF decreased with the increase of cardiac function classification. The observation group had plasma BNP as (273.46 ± 186.58) pg/rrd, LVEF as (35.18 ± 4.73)% before treatment, and plasma BNP as (95.20 ±56.92) pg/ml, LVEF as (43.18± 7.35)% after treatment. The observation group had lower plasma BNP and higher LVEF after treatment than before treatment. Their difference was statistically significant (P〈0.05). Conclusion Plasma BNP level has high application value in diagnosis and prognostic evaluation of chronic heart failure.
