摘要
本研究将284例急性脑梗死患者作为观察组,100例健康人作为对照组,对比两组的颈动脉、椎动脉的血管内径、内膜中膜厚度(intima-media thickness,IMT)、血管阻力指数(resistance index,RI)、收缩期血流速度(systolic velocity,Vs)、舒张期血流速度(diastolic velocity,Vd),发现两组在左颈总动脉内径、右颈总动脉内径、左颈内动脉内径、右颈内动脉内径、左颈总动脉RI、右颈总动脉RI、左颈内动脉RI、左颈总动脉Vd、右颈总动脉Vd、左颈内动脉Vs、右颈内动脉Vs、左颈内动脉Vd、右颈内动脉Vd、右椎动脉Vs、右椎动脉Vd、等指标方面均有差异,观察组动脉内径厚于对照组,血管阻力指数高于对照组,血流速度低于对照组(p<0.05)。进展性脑梗死组及非进展性脑梗死组在左颈内动脉IMT、右颈内动脉IMT、左颈总动脉Vs、左颈总动脉RI、右椎动脉Vs等参数方面有显著差异,进展性脑梗死组的内膜中膜厚度要厚于非进展性脑梗死组(p<0.05)。年龄与颈动脉及椎动脉病变具有一定的相关性(p<0.05)。提示动脉粥样硬化是脑梗死的特征之一,年龄越高,动脉发生病变的可能性越大,IMT增厚可能是脑梗死进展加重的危险信号。
In this study, 284 cases of patients with acute cerebral infarction (ACI) were divided into observation group while 100 cases of healthy people were divided into control group. After contrasting the blood vessel diame- ter, intima-media thickness, resistance index, Systolic velocity, diastolic velocity of carotid and vertebral artery, we found that there were significant differences between the two groups of the letS common carotid artery diameter, the right common carotid artery diameter, letS internal carotid artery diameter, right internal carotid artery diameter, the left common carotid artery RI, the right common carotid artery RI, the letS internal carotid artery RI, the letS common carotid artery of Vd, the right common carotid artery of Vd, the letS internal carotid artery Vs, the right internal carotid artery Vs, the left internal carotid artery of Vd, the right internal carotid artery of Vd, right vertebral artery Vs, right vertebral artery Vd. Artery internal diameter of observation group were thicker than the control group, vas- cular resistance index were higher than the control group, blood flow velocity were lower than the control group (p〈0.05). There were significant differences between the progressive ACI group and non-progressive ACI group of the letS internal carotid artery IMT, the right internal carotid artery IMT, the left common carotid artery of Vs, the left common carotid artery RI, right vertebral artery Vs. Artery internal diameter of progressive ACI group were thicker than non-progressive ACI group (p〈0.05). Age has a certain relevance with pathological changes of carotid and vertebral artery. In conclusion (p〈0.05), carotid atherosclerosis vascular structural decompensation is one of ACI characteristics. The higher the age, the higher the risk of arterial lesions occur. IMT thickening of the internal carotid artery may be a risk signal of ACI progression.
