腹腔镜下两种宫颈癌根治术式对患者术后生活质量的影响比较

Comparison of the effects of two types of laparoscopic radical resection of cervical cancer on patients' postoperative quality of life

目的比较腹腔镜下保留盆腔自主神经的广泛性子宫切除术(Laparoscopic nerve sparing radical hysterectomy,LNSRH)与腹腔镜下广泛性子宫切除术(Laparoscopic radical hysterectomy,LRH)对患者术后生活质量的影响。方法选择2014年3月~2015年11月本院收治的20例宫颈癌患者,采用随机法将20例宫颈癌患者随机分成对照组和观察组,各10例,对照组患者实施常规腹腔镜下广泛子宫切除术,观察组患者实施腹腔镜下保留盆腔自主神经的广泛性子宫切除术,对比两组患者的拔除尿管的平均时间及性功能障碍发生率。结果观察组拔除尿管的平均时间(11.50±2.838)d,对照组拔除尿管的平均时间(19.20±7.714)d,差异有统计学意义(P<0.05)。观察组术后性功能障碍发生率60%,FSFI总评分为(18.57±11.5)分。对照组术后性功能障碍发生率80%,FSFI总评分为(15.90±11.5)分,两组比较,差异无统计学意义(P>0.05)。结论LNSRH可缩短术后留置尿管时间,但对术后性生活质量的提高不明显。

Objective To compare the effect of laparoscopic nerve sparing radical hysterectomy （LNSRH） and laparo- scopic radical hysterectomy （LRH） on patients＇ postoperative quality of life. Methods 20 patients with cervical cancer who were admitted to the hospital from March 2014 to November 2015 were selected. 20 cases of cervical cancer patients were randomly divided into control group and observation group, with 10 cases in each group. The patients in the control group were given conventional laparoscopic radical hysterectomy （LRH）, and patients in the observation group were given laparoscopic nerve sparing radical hysterectomy （LNSRH）. The average time of removal of urinary catheter and the incidence rate of sexual dysfunction were compared between the two groups. Results The average time of removal of urinary catheter in the observation group was （11.50±2.838） days, and the average time of removal of urinary catheter in the control group was（19.20±7.714 ）days, the difference was statistically significant（P〈O.05）. The incidence rate of postoperative sexual dysfunction in the observation group was 60%, and FSFI score was （18.57±11.5）. The incidence rate of postoperative sexual dysfunction in the control group was 80%, and FSFI score was （15.90±11.5）. The total score was compared between the two groups, the difference was not statistically significant（P〉0.05）. Conclusion This study shows that LNSRH can shorten the time of postoperative indwelling catheter, but the improvement of postoperative sexual life treatment is not obvious.
