西安市2000-2016年报告的50岁及以上HIV/AIDS病人特征分析

Characteristics of HIV infections among over 50-year-old population in Xi'an from 2000 to 2016

目的分析西安市≥50岁的艾滋病病毒(HIV)感染者和艾滋病(AIDS)病人(简称HIV/AIDS病人)特征。方法从中国疾病预防控制信息系统的艾滋病综合防治信息系统中获取艾滋病网络直报数据,筛选西安市截至2016年底累计报告年龄≥50岁的HIV/AIDS病例。结果 2000-2016年西安市累计报告≥50岁的HIV/AIDS病例1 031例,2008-2016年病例数依次是15例、18例、38例、43例、103例、129例、177例、233例和252例,差异有统计学意义(χ~2=78.525,P=0.000)。异性传播占63.5%,其中女性占84.7%,男性占59.6%,差异有统计学意义(χ~2=49.848,P=0.000);2008-2016年异性传播比例依次是40.0%、33.3%、52.6%、41.9%、66.0%、65.9%、67.2%、68.7%和66.7%,差异有统计学意义(χ~2=14.812,P=0.000)。医疗机构检测发现男女病例分别占74.0%和70.6%。根据首次CD4+T淋巴细胞检测结果推断37.2%的病例感染时间≥8年,54.4%的病例可能是≥50岁时感染。结论西安市≥50岁HIV/AIDS病例呈增长趋势,在老年人群尤其是老年男性中应加强针对性宣传,提高检测可及性,尽可能早期发现患者。

Objective To analyze the characteristics of HIV/AIDS among over 50-year-old population in Xi＇an from 2000 to 2016.Methods Data on HIV/AIDS cases among over 50-year-old was downloaded from the national HIV prevention and control information system.Results A total of 1031 HIV/AIDS cases among over 50-year-old were reported from 2000 to 2016.The cases got increasing annually from 2008 to 2016（cases of 15,18,38,43,103,129,177,233,and 252χ-2=78.525,P=0.000）.Over 50-year-old population were infected mainly through heterosexual transmission（63.5%）with the female of 84.7% and male of 59.6%（χ-2=49.848,P=0.000）.The heterosexual transmission proportion during 2008-2016 was 40.0%,33.3%,52.6%,41.9%,66.0%,65.9%,67.2%,68.7%,and 66.7%,respectively,increasing annually（χ-2=14.812,P=0.000）.Both of male and female cases were found mainly from medical institutions（74.0% and 70.6%）.37.2% of them had been infected with HIV for more than 8 years based on the first time of CD4 testing results after the diagnosis,and 54.4% were estimated being infected at age of 50 and over.Conclusion The number of HIV/AIDS among over 50-year-old population is increasing rapidly.The elderly people,especially the elderly men,should get strengthened and targeted publicity of AIDS knowledge and detection accessibility to identify patients be improved as early as possible.
