泪小管断裂吻合术中双路硅胶管植入的应用体会

Application of double-passage silicone tube intubation in anastomosis for lacrimal canalicular laceration

目的 对泪道双路硅胶管植入治疗泪小管断裂的效果进行观察,并总结应用体会.方法 选择2015年7月—2017年1月山东省莒县城阳卫生院收治的28例（28眼）泪小管断裂患者,在泪小管断裂吻合术中采用泪道双路硅胶管植入法治疗,对手术效果、泪溢情况、舒适度及外观等进行观察.结果 术后28例患者在日常生活中无明显不适,眼睑外形保持正常,无眼睑内外翻,无明显泪点豁裂,无明显鼻腔异物感,置管期间硅胶管未脱出.28例泪道通畅无溢泪,其中2例泪道冲洗阻力较大并明显溢泪,3例泪道通畅但仍轻度溢泪.结论 在下泪小管断裂吻合术中应用泪道双路硅胶管置管,临床疗效令人满意,该手术方法简单、易行,适合在基层医院开展.

Objective To observe the double-passage silicone tube intubation in anastomosis for lacrimal canalicular laceration and summarize the application experiences. Methods We collected 28 patients with lacrimal canalicular laceration in Chengyang Health Center, Juxian County, Shandong Province, from July, 2015 to January, 2017. All patients adopted the double-passage silicone tube intubation in anastomosis for lacrimal canalicular laceration. The operation effect, epiphora, comfort degree, and appearance were observed. Results All patients have no obvious discomforts, keep normal eyelid, no eyelid entropion and ectropion, no lacrimal punctum gapping, no obvious foreign body sensation in nasal cavity, and no abjection during the cathetering. Moreover, 28 cases had no tears overflow under lacrimal duct flow, lacrimal duct flushing in 2 cases were not free and clear tears overflowed, and 3 cases have mild overflow tears. Conclusion The clinical effect of the double-passage silicone tube intubation in anastomosis for lacrimal canalicular laceration is satisfactory, and this method is deserved to extensive application.
