左西孟旦在缺血性心力衰竭患者PCI术中的应用价值

Application effects of percutaneous coronary intervention assisted by levosimendan in the treatment of ischemic heart failure

目的探讨左西孟旦在缺血性心力衰竭患者冠状动脉介入术(PCI)中的应用价值。方法选取平顶山市第一人民医院2013年1月至2016年1月收治的缺血性心力衰竭行PCI术治疗的患者70例,按照随机数表法分为两组,各35例。对照组患者接受常规对症支持治疗及PCI术治疗,观察组患者在对照组基础上于PCI术前接受左西孟旦治疗,比较两组患者治疗效果。结果观察组患者手术成功率高于对照组(94.3%比68.6%),术后并发症发生率低于对照组(5.7%比22.9%),术后1个月LEVF高于对照组[(52.8±5.3)%比(48.4±3.8)%],6 min步行距离长于对照组[(447.4±28.0)m比(430.4±20.0)m],血肌酐水平低于对照组[(81.4±7.0)μmol/L比(87.0±6.2)μmol/L],差异均有统计学意义(P<0.05)。结论左西孟旦应用于接受PCI术治疗的缺血性心力衰竭患者,可提高患者手术成功率,降低术后并发症发生率,改善患者术后心、肾功能,值得临床推广。

Objective To study the application effects of percutaneous coronary intervention assisted by levosimendan in the treatment of ischemic heart failure. Methods Seventy patients with ischemic heart failure who were treated in the First People＇s Hospital of Pingdingshan from January of 2013 to January of 2016 were enrolled in this study. All patients were randomly divided into two groups, 35 cases in each group. Patients in the control group were treated with PCI on the basis of conventional therapy, and patients in the observation group were treated with levosimendan before PCI on the basis of control group. The clinical effects were compared between observation group and control group. Results Compared with the control group, the observation group had statistically significant higher success rate of surgery （94. 3% vs 68.6% ） , less complications （5.7% vs 22. 9% ） , higher level of LEVF [ （ 52. 8±5.3 ） % vs （48.4±3.8 ） % ], longer distance of 6 - minute walk [ （447. 4±28.0 ） m vs （430. 4±20. 0 ）m ] , lower serum level of creatinine [ （ 81.4±7.0 ）μmol/L vs （ 87. 0±6. 2 ） μmol/L ] （ P 〈 0. 05 ）. Conclusion The application of levosimendan before PCI in the treatment of isehemic heart failure could enhence the success rate of PCI, reduce the rate of complication. The cardiac and renal function of patients are improved significantly after percutaneous coronary intervention assisted by levosimendan, ideal and worthy of populaarization and application.
