血府逐瘀汤治疗急性缺血性脑卒中的疗效及其对患者血清超敏C反应蛋白和血液流变学的影响

Clinical effect of Xuefu Zhuyu Decoction in acute ischemic stroke and its effect on high sensitivity C reactive protein and hemorheology

目的探讨血府逐瘀汤治疗急性缺血性脑卒中的临床疗效及对血清超敏C反应蛋白(hs-CRP)、血液流变学指标的影响。方法选择榆林市星元医院神经内二科于2015年1月至2016年12月期间收治的急性缺血性脑卒中患者80例为研究对象,根据随机数表法将患者分为观察组和对照组,每组40例,对照组予阿司匹林等常规治疗,观察组在此基础上联合应用血府逐瘀汤治疗,疗程均为4周。比较两组患者治疗前后的美国国立卫生研究院卒中量表(NIHSS)评分、血清hs-CRP及血液流变学指标的变化。结果治疗后,观察组患者的NIHSS评分及血清hs-CRP水平分别为(7.81±1.64)分、(6.79±1.57)mg/L,均明显低于对照组的(9.33±2.28)分、(8.99±1.92)mg/L,差异均有统计学意义(P<0.05);观察组患者的血液流变学指标包括全血比黏度(高切、中切、低切)、血浆黏度及纤维蛋白原均明显低于对照组,差异均有统计学意义(P<0.05)。结论急性缺血性脑卒中患者在常规治疗的基础上联合应用血府逐瘀汤可以有效减轻炎症反应,降低血液黏稠度,有利于改善患者预后。

Objective To investigate the clinical effect of Xuefu Zhuyu Decoction in acute ischemic stroke and its effect on serum high sensitivity C reactive protein（hs-CRP） and hemorheology. Methods A total of 80 cases of patients with acute ischemic stroke, who admitted to Department of Neurology Number 2 of Yulin Xingyuan Hospital from January 2015 to December 2016, were selected and divided into the observation group and the control group according to the random number table method, with 40 cases in each group. The control group were given aspirin and other conventional treatment, and the observation group was treated with additional Xuefu Zhuyu Decoction based on the treatment method of the control group. The treatment lasted for 4 weeks. The changes of National Institutes of Health Stroke Scale（NIHSS） score, serum hs-CRP and hemorheology were compared between the two groups before and after treatment. Results After treatment, the NIHSS and serum hs-CRP levels in the observation group were（7.81±1.64） and（6.79±1.57）mg/L, respectively, which were significantly lower than（9.33 ± 2.28） and（8.99 ± 1.92） mg/L in the control group（P〈0.05）; the hemorheology including whole blood viscosity（high-cut, medium and low-cut）, plasma viscosity, hematocrit,fibrinogen in the observation group were significantly lower than those in the control group（P〈0.05）. Conclusion For patients with acute ischemic stroke, the application of Xuefu Zhuyu Decoction on the basis of conventional treatment can effectively reduce the inflammatory response, reduce blood viscosity, and improve the prognosis of patients.
