益肺健脾方联合布地奈德雾化吸入对慢性阻塞性肺疾病患者肺功能、T淋巴细胞及血流动力学的影响

Influence of Lung-Benefiting and Spleen-Strengthening Prescription Combined with Budesonide Aerosol Inhalation on the Lung Function,T lymphocyte and Hemodynamics in Patients with Chronic Obstructive Pulmonary Disease

目的:观察益肺健脾方联合布地奈德雾化吸入对慢性阻塞性肺疾病(Chronic obstructive pulmonary disease,COPD)患者肺功能、T淋巴细胞及血流动力学的影响。方法:选取2015年12月—2017年1月本院收治的COPD患者116例作为研究对象,随机分为观察组和对照组,每组58例。对照组给予布地奈德雾化吸入治疗,观察组在对照组治疗的基础上加用益肺健脾方治疗。观察两组患者中医证候积分、临床疗效、第一秒末用力呼气量(FEV1)、气道阻力(Raw)、肺活量(VC)、每分钟最大通气量(MVV)、T淋巴细胞水平以及血流动力学。结果:观察组治疗后中医证候积分显著低于对照组(P<0.05);观察组有效率91.38%,对照组有效率75.86%,观察组优于对照组(P<0.05);观察组治疗后FEV1、VC、MVV、Raw优于对照组(P<0.05);观察组治疗后CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+优于对照组(P<0.05);观察组治疗后全血黏度高切值、全血黏度低切值、纤维蛋白原、血浆黏度优于对照组(P<0.05)。结论:益肺健脾方联合布地奈德雾化吸入可有效改善COPD患者肺功能和T淋巴细胞水平,提高临床疗效。

Objective：To observe the influence of Lung-Benefiting and Spleen-Strengthening Prescription combined with Budes- onide aerosol ild^alation on the lung function ,T lymphocyte and hemodynamics ill patients with chronic obstructive pulmonar3r dis- ease （COPD）. Methods：Totally 116 patients with COPD treated in this hospital between December 2015 and January. 2017 were randomized into observation group and control group,58 cases in each group. Control group were given Budesonide aerosol inhala- tion while observation group were added with Lung-Benefiting and Spleen-Strengthening Prescription. The TCM symptom scores, clinical effects, FEVI , Raw,VC, MVV,level of T lymphocyte and hemodynamics in the two groups were observed. Results ： After treatment,the TCM symptom scores in observation group were markedly lower than those in control group （ P 〈 0.05 ）. The effec- tive rate was 91.38% in observation group and 75.86% ;observation group were superior to control group （ P 〈 0.05 ）. The levels of FEVt, Raw,VC, MVV, CD3^ ＋ , CD4 ^＋ , CD8^＋ and CD4^ ＋/CD8^＋ in observation group were superior to those in control group （ P 〈 0.05 ）. The whole blood viscosity （high and low shearing） , fibrinogen and plasma viscosity in observation group were superior to those in control group （ P 〈 0. 05 ）. Conclusion ： Lung-Benefiting and Spleen-Strengthening Prescription combined with Budes- onide aerosol inhalation can effectively improve the lung function and level of T lymphocyte in patients with COPD and thus en- hance the clinical effects.
