益肾降浊方联合苯溴马隆治疗尿酸性肾病临床研究

Clinical Study on Treatment of Uric Acid Nephropathy with Yishen Jiangzhuo Fang Combined with Benzyl Bromide Malone

目的:观察自拟益肾降浊方联合苯溴马隆治疗尿酸性肾病的临床疗效。方法:将60例尿酸性肾病患者随机分为治疗组和对照组,每组30例。对照组采用苯溴马隆治疗,治疗组在对照组治疗基础上加用益肾降浊方(萆薢12 g,黄芪30 g,熟地黄20 g,党参10 g,土茯苓20 g,秦艽100 g,车前子15 g,丹参20 g,苍术10 g,生白术15 g,炒薏苡仁15 g,牛膝15 g,川芎12 g)治疗,两组患者均治疗3个月,治疗后评价临床疗效、治疗前后血尿酸、血尿素氮(blood urea nitrogen,BUN)、肌酐(creatinine,Cr)、血清胱抑素C的变化。结果:对照组有效率为73%;治疗组有效率为90%,两组比较,差异有统计学意义(P<0.05);治疗后,治疗组的血尿酸较对照组明显降低,差异有统计学意义(P<0.05)。治疗前后BUN、Cr、血清胱抑素C较对照组均明显降低,差异有统计学意义(P<0.05)。治疗前后肝功能、蛋白尿比较,差异均无统计学意义(P>0.05)。结论:自拟益肾降浊方联合苯溴马隆是治疗尿酸性肾病的有效方法,能明显降低血尿酸水平,改善患者肾功能,延缓尿酸性肾病的疾病进展。

Objective： To observe the clinical effect of Yishen Jiangzhuo Fang combined with benzbromarone in the treatment of uric acid nephropathy. Methods： 60 patients with uric acid nephropathy were randomly divided into treatment group and control group,with 30 cases in each group. The control group was given benzbromarone,while the treatment group was added Yishen Jiangzhuo Fang on the basis of therapy of the control group,the composition： Bixie（ Yam Rhizome） 12 g,Huangqi（ Radix Astragali） 30 g,Shudihuang（ Cooked Rehmannia） 20 g,Dangshen（ Codonopsis Pilosula） 10 g,Tufuling（ Glabrous Greenbrier Rhizome） 20 g,Qinjiao（ Radix Gentianae Macrophyllae） 100 g,Cheqianzi（ Semen Plantaginis） 15 g,Danshen（ Salvia Miltiorrhiza） 20 g,Cangzhu（ Rhizoma Atractylodis） 10 g,Shengbaizhu（ Atractylodes） 15 g,Caoyiyiren（ Fried Semen Coicis） 15 g,Niuxi（ Radix Achyranthis Bidentatae） 15 g,Chuanxiong（ Rhizoma Ligustici Wallichii） 12 g. The two groups were treated for 3 months,and the clinical efficacy was evaluated after treatment. The changes of blood uric acid,BUN,creatinine（ Cr） and serum cystatin C were evaluated after treatment. Results： The effective rate of the control group was 73% and the effective rate of the treatment group was 90%. The difference was statistically significant（ P 0. 05）. After treatment,the blood uric acid in the treatment group decreased significantly compared with the control group,and the difference was statistically significant（ P 0. 05）. Compared with the control groupbefore and after treatment,there was a significant decrease in the BUN,Cr and serum cystatin C and the difference was statistically significant（ P 0. 05）. Before and after treatment,there was no statistically significant difference between liver function and proteinuria（ P 0. 05）. Conclusion： Yishen Jiangzhuo Fang combined with benzbromaronean is an effective method for the treatment of uric acid nephropathy,which can significantly reduce the blood uric acid level,improve the renal function of patients,and delay the disease progression of uric acid nephropathy.
