宫腔镜联合阴道高频彩超对子宫内膜癌的诊断价值

Diagnostic Value of Hysteroscopy Combined with Vaginal High Frequency Ultrasonography in the Diagnosis of Endometrial

目的探讨宫腔镜联合阴道高频彩超对子宫内膜癌的诊断价值。方法选取120例疑似子宫内膜病变患者为研究对象。所有患者均进行阴道高频彩超、宫腔镜检查,以经穿刺或手术病理活检的结果为参考标准,作为对照组,并将行宫腔镜联合阴道高频彩超诊断的患者作为观察组,将行阴道高频彩超检查诊断的患者作为阴道高频彩超组。对比各组的诊断结果,并分析宫腔镜联合阴道高频彩超对子宫内膜癌诊断的准确率、特异度、灵敏度、阴性预测率、阳性预测率。结果对照组与阴道高频彩超组的子宫内膜癌的诊断结果相比,差异有统计学意义(P<0.05)。对照组与观察组的子宫内膜癌的诊断结果相比,差异没有统计学意义(P>0.05)。观察组的符合率、特异度、灵敏度、阴性预测率、阳性预测率均优于阴道高频彩超组,差异具有统计学意义(P<0.05)。结论宫腔镜联合阴道高频彩超能够准确地对子宫内膜癌做出判断,有较高的特异度、灵敏度、阴性预测率以及阳性预测率。

Objective To evaluate the diagnostic value of hysteroscopy combined with vaginal high frequency uhrasonog- raphy for endometrial carcinoma. Methods 120 cases of suspected endometrial lesions were selected as the research object. All patients were subjected to vaginal high frequency ultrasonography, hysteroscopy, puncture or surgical biopsy results as a reference standard, as the control group, and hysteroscopy combined with vaginal high frequency color doppler ultrasound diagnosis of patients as the observation group, vaginal high frequency color doppler ultrasonography in patients diagnosed as vaginal high frequency ultrasonography. To compare the diagnostic results of hysteroseopy combined with vaginal high frequency uhrasonography in the diagnosis of endometrial carcinoma, the specificity, sensitivity, negative predictive rate and positive predictive rate were compared. Results Compared with the diagnosis of endometrial carcinoma, the difference was statistically significant （ P 〈 0.05 ）. There was no statistically significant difference in the diagnostic results of endoscopic carcinoma between the 2 groups （P 〉 0.05 ）. The coincidence rate, specificity, sensitivity, negative predictive rate and positive predictive rate of the observation group were better than those of the vaginal high frequency group. The difference was statistically significant （ P 〈 0.05 ）. Conclusion Hysteroscopy combined with vaginal high frequency color doppler ultrasound can accurately judge the endometrial carcinoma,with higher speci- ficity, sensitivity, negative predictive rate and positive predictive rate.
