胃炎患者血浆凝血指标改变的临床意义

The variation of coagulation function tests in patients with gastritis

目的 探讨胃炎患者血浆凝血指标的改变及其临床意义.方法 分别对2015年6月至2016年12月间空军总医院消化内科收治的1262例胃炎患者、140例胃癌患者以及进行健康体检的1132例正常对照进行血浆凝血酶原时间（PT）、活化部门凝血活酶时间（APTT）、凝血酶时间（TT）、纤维蛋白原含量（FIB）以及血小板参数（PLT、PCT、MPV、PDW）的检测,比较并分析胃炎患者血浆凝血指标的临床改变.结果 胃炎患者APTT 指标33.4 ± 3.3（s）与健康对照32.0 ± 2.2（s）相比明显延长,差异有统计学意义（P〈0.001）;PT、TT与健康对照相比稍延长（P〈0.05）,FIB与健康对照相比无明显差异（P〉0.05）.胃癌患者APTT比健康对照显著缩短,PT、T T、FIB比健康对照显著延长,差异有统计学意义（P〈0.001）;胃炎患者的血小板参数与健康对照相比没有明显差异（P〉0.05）;胃炎患者APTT与血小板参数（PLT、PCT、MPV、PDW）之间均不具有相关性（P〉0.05）.结论 胃炎患者APTT指标比健康对照显著延长,具有敏感性优势;凝血检测对于胃炎的辅助诊断具有一定临床价值.

Objective To investigate the variation of coagulation function tests in patients with gastritis.Methods We collected 1262 patients of gastritis,140 patients of gastric cancer and 1132 cases of healthy control during the 2015-2016 in Air Force General Hospital,PLA.Tests of prothrombin time,activated partial thromboplastin time,fibrinogen,thrombin time,platelet count,thrombo-cytocrit,mean platelet volume,platelet distribution width were conducted on blood samples from these subjects.Results The APTT of gastritis group （33.4 ± 3.3）was obviously higher than that of healthy control （32.0 ± 2.2）and the difference was statis-tically significant（P〈0.001）.PT and TT tests of gastritis group were slightly higher than that of healthy control （P〈0.05）. There is no statistical difference in FIB content between gastritis group and healthy control （P〉0.05）.APTT of GC group was lower than that of healthy control （P〈0.001）.Meanwhile,PT,TT,FIB of GC group was obviously higher than that of healthy control（P〈0.001）.There is no difference in PLT parameters between gastritis group and healthy control （P〉0.05）.No correla-tion was observed between the APTT and PLT parameters （PLT、PCT、MPV、PDW）（P〉0.05）.Conclusion The APTT of gas-tritis group was obviously higher than that of healthy control.The coagulation function tests are clinically important to gastritis pa-tients.
