手术治疗老年患者肺大疱并发自发性气胸的疗效分析

Surgical treatment of pulmonary bulla complicated with spontaneous pneumothorax in elderly patients

目的 探讨手术治疗老年患者肺大疱并发自发性气胸的临床疗效.方法 选取2016年3月至2017年3月在本院接受治疗的30例肺大疱并发自发性气胸老年患者进行研究,将所有患者随机分为观察组以及对照组,每组15例,对照组予以胸腔闭式引流术保守治疗,观察组予以胸腔镜治疗,实验结束后,对两组患者手术时间、术中出血量、术后引流量、住院时间、引流时间、镇痛剂使用时间、手术前后肺功能恢复状况以及并发症发生率进行比较分析.结果 观察组患者手术时间、术中出血量、术后引流量、住院时间、引流时间、镇痛剂使用时间以及并发症发生率明显低于对照组,肺功能恢复状况明显高于对照组,P〈0.05.结论 相对于胸腔闭式引流术保守治疗来说,胸腔镜治疗老年患者肺大疱并发自发性气胸的疗效显著,安全性较高,可有效改善患者肺功能状况,缩短住院时间,减轻患者经济负担,值得临床推广应用.

Objective To investigate the clinical efficacy of surgical treatment of elderly patients with pulmonary bullae compli-cated with spontaneous pneumothorax.Methods 30 cases of pulmonary bullae complicated with spontaneous pneumothorax in eld-erly patients from March 2016 to March 2017 in our hospital were studied.All patients were randomly divided into observation group and control group,15 cases in each group,the control group received chest closed drainage of conservative treatment,the ob-servation group received thoracoscopic treatment,after the end of the experiment,the two groups of patients with operation time,in-traoperative bleeding,postoperative drainage,hospitalization time,drainage time,analgesic use,the recovery of lung function before and after surgery and the incidence of complications Were compared.Results Patients with operation time,intraoperative bleeding, postoperative drainage,hospitalization time,drainage time,analgesic use time and complication rate was significantly lower than the control group,lung function recovery was significantly higher than control group,P〈0.05.Conclusion Compared with closed tho-racic drainage,conservative treatment and the effect of thoracoscopic treatment of elderly patients with pulmonary bullae complicat-ed with spontaneous pneumothorax is significant,high safety,can effectively improve the pulmonary function of patients,shorten the hospitalization time,reduce the economic burden of the patients,it is worthy of clinical application.
