社区卫生服务中心糖尿病患者首诊情况及影响因素调查研究

Investigation on the Initial Visit of Diabetic Patients to Community Health Service Center and Influencing Factors

目的了解社区卫生服务中心门诊糖尿病患者首次就诊情况,探讨影响糖尿病患者选择社区卫生服务中心作为首诊医疗机构的相关因素,为进一步做好社区首诊工作、推进双向转诊提供参考。方法选择2015年1—12月期间在广东省佛山市某社区卫生服务中心门诊就诊的2型糖尿病患者300例,对其就诊特点等内容进行问卷调查,采用多因素Logistic回归对影响因素进行分析。结果 300例2型糖尿病患者中,首诊选择在社区卫生服务中心的共116例(38.7%)。就诊意愿(OR=3.89)、交通方式(OR=4.60)、候诊时间(OR=2.13)是糖尿病患者选择社区卫生服务中心作为首诊医疗机构的主要影响因素。结论糖尿病患者首诊选择社区卫生服务中心的就诊率不高,应进一步增强患者的就诊意愿、加强社区卫生服务的便捷性和可及性等,以提高患者社区首诊就诊率。

Objectives This article intends to investigate the initial visit of diabetic patients to the Community Health Service Center（CHSC）,to discuss relating factors influencing patients＇ choice of the CHSC as their initial health service institution,therefore to provide reference for promoting the work of first diagnosis in community and two-way referral.Methods 300 patients with typeⅡ diabetes visiting a CHSC in Foshan,Guangdong in 2015 were selected.A questionnaire survey on the clinical characteristic of the visit was conducted among them and the Multivariate Logistic Regression was applied for the analysis of the influencing factors.Results Out of the 300 cases of patients with typeⅡ diabetes,116（38.7%）were first diagnosed in CHSC.Multivariate Logistic regression analysis showed that treatment will（OR=3.89）,transportation（OR=4.60）,waiting time（OR=2.13）were the main factors influencing patients＇ choice of the CHSC as their initial health service institution.Conclusions The ratio of diabetic patients choosing CHSC as their initial health service institution is low.Further measures should be put forward to encourage patients＇ willingness to visit CHSC,make community health service more convenient and accessible,so as to ratio of initial visit in the community.
