摘要
目的探讨颅脑损伤后血浆血管紧张素(angiotensin)水平与脑血管痉挛发生的相关关系。方法收集我院2015-01—2016-01收治的颅脑损伤患者238例,检测入院后第1、3、7、14、21、28天患者血液及脑脊液中血管紧张素的水平,相同时间点应用经颅多普勒超声(TCD)检查患者颅内血管的血流速度,以反映患者脑血管痉挛情况,研究两者之间的相关关系。结果238例患者血浆血管紧张素浓度与各脑血管血流速度呈正相关性,相关系数分别为0.58、0.72、0.66、0.51;血浆中血管紧张素在患者颅脑损伤后即开始快速上升,于第7天达到高峰,后缓慢下降。脑血管的血流速度在患者颅脑损伤后即开始快速上升,于第7天达到高峰,后缓慢下降。结论颅脑损伤后血浆血管紧张素水平与脑血管痉挛发生存在显著的相关关系。提示血管紧张素在颅脑损伤患者脑血管痉挛发生的过程中发挥着重要的作用。
Objective To investigate the relevance between angiotensin in plasma and cerebral vasospasm after traumatic brain injury .Methods 238 patients with traumatic craniocerebral injury treated from January 2015 to January 2015 were collected , and tested plasma angiotensin on 1st ,3th ,7th ,14th ,21th ,28th days ,at the same time the velocity of intracranial vascular blood flow were examined by the application of transcranial doppler ultrasound to reflect cerebral vasospasm .The relationship between the angiotensin and cerebral vasospasm were compared .Results There was a positively correlation between the cerebral blood flow velocity and plasma angiotensin concentrations of 238 patients ,and the correlation coefficients were 0.58 ,0.72 ,0.66 ,0.51 ;After craniocerebral injury plasma angiotensin began to rise rapidly ,peaked on 7th day ,and then slowly declined .Cerebral blood flow velocity in patients after craniocerebral injury has been rising ,peaked on 7th day ,and then slowly declined .Conclusion After traumatic craniocerebral injury there is significant correlation between cerebral vasospasm and plasma angiotensin .Plasma angio-tensin plays an important role in the process of cerebral vasospasm after traumatic craniocerebral injury .
