椎弓根螺钉联合椎间融合术治疗腰间盘突出的临床疗效观察

Observation on clinical effect of pedicle screw combined with interbody fusion in the treatment of lumbar disc herniation

目的探讨椎弓根螺钉联合椎间融合术治疗腰间盘突出的临床疗效。方法 60例腰间盘突出患者,按照治疗方法的不同分为观察组和对照组,每组30例。对照组予以椎间融合术治疗,观察组予以椎弓根螺钉联合椎间融合术治疗,实验结束后,对两组患者的视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分、腰椎日本骨科学会(JOA)评分以及生活质量评分进行比较分析。结果观察组患者JOA评分(22.03±2.79)分明显高于对照组的(20.01±2.52)分,VAS评分(2.33±1.06)分以及ODI评分(9.45±2.58)分明显低于对照组的(3.21±1.15)、(11.23±2.64)分,差异均具有统计学意义(t=2.943、3.082、2.641,P<0.05)。观察组患者生活质量(生理职能、情感职能、社会功能、精神健康)评分明显高于对照组,差异具有统计学意义(P<0.05)。结论腰间盘突出患者采用椎弓根螺钉联合椎间融合术治疗效果显著,安全性较高,可有效改善患者的腰椎功能,提高其生活质量,值得临床推广应用。

Objective To discuss the clinical efficacy of pedicle screw combined with interbody fusion in the treatment of lumbar disc herniation.Methods A total of 60 lumbar disc herniation patients were divided by different treatment methods into observation group and control group,with 30 cases in each group.The control group was treated with interbody fusion,and the observation group was treated with pedicle screw combined with interbody fusion.After experiment,comparison and analysis were made on visual analogue scale（VAS） score,Oswestry disability index（ODI） score,lumbar Japanese Orthopaedic Association（JOA） score and quality of life score between two groups.Results The observation group had obviously higher JOA score as（22.03±2.79） points than（20.01±2.52） points in the control group,and obviously lower VAS score as（2.33±1.06） points and ODI score as（9.45±2.58） points than（3.21±1.15） and（11.23±2.64） points in the control group.Their difference was statistically significant（t=2.943,3.082,2.641,P0.05）.The observation group had obviously higher quality of life（physiological function,emotional function,social function,mental health） score than the control group,and their difference was statistically significant（P0.05）.Conclusion Combination of pedicle screw and interbody fusion shows excellent effect for lumbar disc herniation patients with high safety,and can effectively improve lumbar function of patients and improve their quality of life.It is worthy of clinical promotion and application.
