胶圈套扎术联合外痔切剥术治疗混合痔的临床效果观察

Clinical observation of rubber ring ligation combined with external hemorrhoids cutting and stripping in the treatment of mixed hemorrhoids

目的探讨胶圈套扎术联合外痔切剥术治疗混合痔的临床效果。方法 78例混合痔患者,随机分为对照组和观察组,每组39例。对照组患者采用外痔切剥术治疗,观察组患者在对照组基础上采用胶圈套扎术治疗。比较两组患者临床疗效、疼痛程度、水肿情况、住院时间和创伤愈合时间。结果观察组患者治疗总有效率为100.00%,与对照组的100.00%比较差异无统计学意义(P>0.05)。观察组患者住院时间为(6.80±2.25)d、创伤愈合时间为(14.77±4.56)d,明显短于对照组的(12.22±3.68)、(23.47±4.66)d,差异有统计学意义(P<0.05)。观察组患者Ⅲ度疼痛和Ⅱ度水肿例数均少于对照组,差异具有统计学意义(P<0.05)。结论胶圈套扎术联合外痔切剥术治疗混合痔疗效确切,能有效减轻术后水肿和疼痛程度,缩短患者住院时间和创伤愈合时间,改善患者预后,值得临床推广使用。

Objective To investigate the clinical effect of rubber ring ligation combined with external hemorrhoids cutting and stripping in the treatment of mixed hemorrhoids.Methods A total of 78 mixed hemorrhoids patients were randomly divided into control group and observation group,with 39 cases in each group.The control group was treated with external hemorrhoids cutting and stripping,and the observation group was treated with rubber ring ligation on the basis of the control group.Comparison were made on clinical efficacy,pain degree,edema condition,hospitalization time and wound healing time between two groups.Results The observation group had no statistically significant difference in total treatment effective rate as 100.00%,comparing with 100.00% in the control group（P0.05）.The observation group had hospitalization time as（6.80±2.25） d,wound healing time as（14.77±4.56） d,which were obviously shorter than（12.22±3.68） and（23.47±4.66） d in the control group,and their difference was statistically significant（P0.05）.The observation group had less Ⅲ degree of pain and Ⅱ degree of edema cases than the control group,and their difference was statistically significant（P0.05）.Conclusion Combination of rubber ring ligation and external hemorrhoids cutting and stripping shows affirmative efficacy,and can effectively reduce postoperative edema and pain degree,shorten hospitalization time and wound healing time,and improve the prognosis of patients.It is worthy of clinical promotion and application.
