天麻钩藤颗粒治疗前庭性偏头痛的临床疗效及其对眩晕复发的预防作用

Clinical Effect of Tianma-Gouteng Granules on Patients with Vestibular Migraine and the Preventive Effect on Recurrent Vertigo

目的观察天麻钩藤颗粒治疗前庭性偏头痛(VM)的临床疗效,并探讨其对眩晕复发的预防作用。方法选取2015年1月—2016年2月上海市嘉定区安亭医院收治的VM患者60例,根据治疗方法分为盐酸氟桂利嗪组与天麻钩藤组,每组30例。盐酸氟桂利嗪组患者给予盐酸氟桂利嗪胶囊治疗,天麻钩藤组患者给予天麻钩藤颗粒治疗;两组患者均持续治疗4周。比较两组患者临床疗效,治疗前、治疗4周后中医证候积分,治疗前、治疗4周后、治疗3个月后眩晕障碍量表(DHI)评分,记录两组患者治疗3个月后眩晕复发情况及治疗期间不良反应发生情况。结果天麻钩藤组患者临床疗效优于盐酸氟桂利嗪组(P<0.05)。治疗前两组患者中医证候积分比较,差异无统计学意义(P>0.05);治疗4周后天麻钩藤组患者中医证候积分低于盐酸氟桂利嗪组(P<0.05)。治疗前两组患者DHI评分比较,差异无统计学意义(P>0.05);治疗4周后、治疗3个月后DHI评分低于盐酸氟桂利嗪组(P<0.05)。治疗3个月后,天麻钩藤组患者眩晕复发率低于盐酸氟桂利嗪组,眩晕复发次数少于盐酸氟桂利嗪组,眩晕复发持续时间短于盐酸氟桂利嗪组(P<0.05)。两组患者治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论天麻钩藤颗粒治疗VM的临床疗效确切,具有一定的眩晕复发预防作用,且安全性较高。

Objective To observe the clinical effect of Tianma-Gouteng Granules on patients with vestibular migraine,to investigate the preventive effect on recurrent vertigo. Methods From January 2015 to February 2016,a total of 60 patients with vestibular migraine were selected in Anting Hospital of Jiading District, Shanghai, and they were divided into control group and observation group according to the therapeutic methods,each of 30 cases. Patients of control group received flunarizine hydrochloride capsules, while patients of observation group received Tianma-Gouteng Granules; both groups continuously treated for 4 weeks. Clinical effect,TCM symptoms score before treatment and 4 weeks after treatment,Dizziness Handicap Inventory（ DHI） score before treatment,4 weeks and 3 months after treatment were compared between the two groups, meanwhile vertigo recurrence conditions 3 months after treatment and adverse reactions during the treatment was recorded,respectively. Results Clinical effect in observation group was statistically significantly better in control group（ P〈0. 05）. No statistically significant differences of TCM symptoms score was found between the two groups before treatment（ P〉0. 05）,while TCM symptoms score in observation group was statistically significantly lower than that in control group 4 weeks after treatment（ P〈0. 05）. No statistically significant differences of DHI score was found between the two groups before treatment（ P〉0. 05）,while DHI score in observation group was statistically significantly lower than that in control group 4 weeks and 3 months after treatment（ P〈0. 05）. After 3 months of treatment,recurrence rate of vertigo in observation group was statistically significantly lower than that in control group, recurrent frequency of vertigo in observation group was statistically significantly less than that in control group,duration of recurrent vertigo in observation group was statistically significantly shorter than that in control group（ P〈0. 05）. No statistically significant differences of adverse reactions was found between the two groups during the treatment（ P〉0. 05）. Conclusion Tianma-Gouteng Granules has certain clinical effect in treating vestibular migraine,has certain preventive effect on recurrent vertigo,too,with relatively high safety.
