健康教育路径管理对乳腺癌PICC置管患者的效果评价

THE EFFECT OF HEALTH EDUCATION PATH MANAGEMENT ON PATIENTS WITH BREAST CANCER UNDERGOING PICC APPLICATION

目的探讨健康教育路径管理在乳腺癌患者PICC置管护理中的应用效果及其评价。方法以2012年1月~2014年12月在临沂市人民医院甲状腺乳腺科行PICC置管实施健康教育路径管理的1034例乳腺癌患者作为实验组;将2007年1月~2011年12月在临沂市人民医院甲状腺乳腺科行PICC置管常规护理的933例乳腺癌患者作为对照组。观察并记录两组并发症的发生情况,并在患者治疗结束拔管时对患者进行护理满意度调查分析。结果实验组发生导管堵塞22例,静脉炎16例,导管移位12例,穿刺口感染18例,接触性皮炎27例,并发症发生率为9.19%;对照组发生导管堵塞38例,静脉炎36例,导管移位23例,穿刺口感染38例,接触性皮炎63例,并发症发生率为21.22%。实验组并发症发生率低于对照组,差异有统计学意义(P<0.05)。实验组满意率为95.55%,对照组满意率为81.03%,实验组满意率高于对照组,差异有统计学意义(P<0.05)。结论在乳腺癌患者PICC置管护理中应用健康教育路径管理,能显著减少患者置管并发症,患者满意率高,提高了患者生存质量。真正体现"一针穿刺,全程治疗"的理念,值得在临床上推广应用。

Objective To evaluate the effect of health education path management on patients with breast cancer undergoing PICC application.Methods 1034 patients with breast cancer who accepted PICC from January,2012 to November,2014 in this hospital were treated with health education path management and formed the experimental group.Another 933 cases of breast cancer who accepted PICC from January,2007 to November,2011 in this hospital were treated with routine nursing and formed the control group.The incidence of complications was observed and recorded and the patients' satisfaction was analyzed after the treatments.Results In the experimental group,22 cases of catheter blockage,16 cases of phlebitis,12 cases of catheter displacement,18 cases of puncturing site infection and 27 cases of contact dermatitis were found,with the incidence of complications was 9.19%.In the control group,38 cases of catheter blockage,36 cases of phlebitis,23 cases of catheter displacement,38 cases of puncturing site infection and 63 cases of contact dermatitis were found,with the incidence of complications was 21.22%(P <0.05).The satisfaction rate was 95.55%in the experimental group and 81.03%in the control group(P <0.05).Conclusion The health education path management can produce good effect on patients with breast cancer undergoing PICC application.
