CD_3、CD_4及CD_8在早发型新生儿败血症诊断中的临床价值

Clinical value of CD_3,CD_4,CD_8 in the diagnosis of early-onset neonatal sepsis

目的:研究并探讨CD_3、CD_4、CD_8在早发型新生儿败血症诊断中的临床价值。方法:选取2014年1月—2015年6月产科出生的40例败血症新生儿、40例局部感染新生儿,分别设为败血症组和局部感染组,另将同期在产科出生且因高胆红素血症住院的50例非感染新生儿设为对照组,所有新生儿均采集外周静脉血,接受血培养检查,对三组新生儿的外周血CD_3、CD_4、CD_8、CD_(64)表达水平进行检测,比较三组新生儿外周血CD_3、CD_4、CD_8、CD_(64)表达水平,并比较CD_(64)与血培养对败血症的检出情况。结果:三组新生儿的外周血CD_3、CD_4、CD_(64)表达水平比较,差异均有统计学意义(P<0.05),而三组新生儿的外周血CD_8表达水平比较,差异无统计学意义(P>0.05);CD_(64)对新生儿早发型败血症的阳性检出率明显高于血培养(P<0.05)。结论:CD_3、CD_4、CD_8等T细胞亚群指标可以灵敏反映新生儿早发型败血症,与CD_(64)联合检测可为新生儿早发型败血症的早期诊断提供借鉴和帮助。

Objective: To study and evaluate the clinical value of CD_3、CD_4、CD_8 in the diagnosis of early-onset neonatal sepsis. Methods: All 40 cases of neonatal septicemia and 40 cases of neonatal infection in hospital from January 2014 to June 2015 were selected as the research object,and the patients were set as septicemia group and local infection group. Fifty patients with non-infected neonates who were hospitalized and highly bilirubinemia were enrolled in this study. All neonates were collected with peripheral blood and blood culture. The expression of CD_3、CD_4、CD_8 and CD_(64) in peripheral blood and were detected by immunohistochemistry. The expression of CD_3、CD_4、CD_8 and CD_(64) in peripheral blood of three groups were compared,and the detection of CD_(64) and blood culture were compared. Results: The expression levels of CD_3、CD_4 and CD_(64) in peripheral blood of three groups of neonates were statistically different( P < 0. 05),but there was no significant difference between the two groups( P > 0. 05). The positive rate of CD_(64) in neonatal early-onset sepsis was significantly higher than that in blood culture( P < 0. 05). Conclusion: CD_3、CD_4、CD_8 and other T cell subpopulation indicators can be sensitive to neonatal early-onset sepsis,and CD_(64) combined detection for early diagnosis of neonatal early-onset sepsis provide reference and help.
