摘要 目的:探讨甲状腺癌手术发生甲状旁腺损伤的危险因素。方法:收治甲状腺癌手术患者79例,回顾性分析发生甲状旁腺损伤的相关危险因素。结果:单因素分析表明行淋巴结清扫、误切、患桥本甲状腺炎、血运障碍及钳夹挫伤是致甲状旁腺损伤的主要危险因素(P<0.05)。多因素分析表明钳夹挫伤及误切是发生甲状旁腺损伤的独立危险因素。结论:在甲状腺癌手术中,选择合理的术式、准确辨认甲状旁腺、精细化操作、完整保留甲状旁腺血供,能够降低甲状旁腺损伤概率。 Objective:To investigate the risk factors of parathyroid injury in thyroid cancer surgery.Methods:79 patients with thyroid cancer were treated,and the related risk factors of parathyroid injury were analyzed retrospectively.Results:Single factor analysis on lymph node dissection,wrong cut,Hashimoto's thyroiditis,blood disorder and clamp contusion were the main risk factors for parathyroid injury(P<0.05).Multivariate analysis showed that clamp,contusion and miss cut were independent risk factors of parathyroid injury.Conclusion:In thyroid cancer surgery,the choice of a reasonable surgical method,accurate identification of parathyroid,meticulous operation,complete preservation of parathyroid blood supply,can reduce the risk of parathyroid injury.
