血清基质金属蛋白酶9和CC型趋化因子18对胰腺癌诊断性能评价 被引量:5

Value of MMP9 and CCL18 in diagnosis of pancreatic cancer

摘要 目的探讨胰腺癌血清中基质金属蛋白酶9(MMP9)和CC型趋化因子18(CCL18)对其是否具有诊断价值,并分析两指标之间的相关性。方法酶联免疫吸附法(ELISA)检测68例胰腺癌患者和104例健康体检者血清中MMP9和CCL18浓度,并分析二者相关性。结果胰腺癌组血清中MMP9和CCL18含量显著高于健康体检者,差异有统计学意义(P<0.05);低分化组MMP9和CCL18浓度与中高分化者比较差异无统计学意义(P>0.05);Ⅲ期~Ⅳ期患者血清中两分子水平均高于Ⅰ期~Ⅱ期患者,差异有统计学意义(P<0.05);淋巴结转移者血清MMP9和CCL18水平亦显著高于未转移者;Pearson相关性分析结果提示血清中CCL18与MMP9含量呈正相关(r=0.49,P<0.05)。结论胰腺癌患者血清MMP9和CCL18可能作为其诊断指标,与淋巴结转移和临床分期具有相关性;胰腺癌侵袭及转移与肿瘤巨噬细胞分泌的CCL18有着重要联系。 Objective To evaluate the diagnostic value of serum matrix metalloproteinase 9( MMP9) and CCL18 in patients with pancreatic cancer,and to analyze the correlation between the two indexes. Methods The levels of serum MMP9 and CCL18 in patients with pancreatic cancer and normal controls were detected by enzyme linked immunosorbent assay( ELISA),and the correlation of them was analyzed. Results The concentrations of serum MMP9 and CCL18 in the patients with pancreatic cancer were obvious higher than normal controls,with the differences statistically significant( P〈 0. 05). And the expression of MMP9 and CCL18 in the pancreatic cancer with lymph node metastasis were obviously higher than non metastasis,with the differences not statistically significant( P〉 0. 05). The serum levels of them in patients at stage Ⅲ-Ⅳ were higher than those at stage Ⅰ-Ⅱ,with the difference statistically significant( P〈 0. 05); and the levels of serum MMP9 and CCL18 in patients with lymph node metastasis were also significantly higher than those without lymph node metastasis. Pearson correlation results suggested that there was a positive correlation between serum CCL18 and MMP9( r = 0. 49,P〈 0. 05). Conclusion Serum levels of MMP9 and CCL18 in patients with pancreatic cancer may serve as diagnostic indicators of lymph node metastasis and clinical stage. The invasion and metastasis of pancreatic cancer are closely related to the secretion of CCL18 by tumor macrophages.
