方剑乔教授从“瘀”“虚”角度辨识慢性疼痛病机 被引量:14

A Novel Explanation of Professor FANG Jianqiao on the Pathogenesis of Chronic Pain Based on “Stagnation” and “Deficiency”

摘要 [目的]总结方剑乔教授对慢性疼痛"虚瘀交错"病机理论的辨识及其临床应用,以更好地传承老中医学术思想。[方法]通过专家深度访谈、跟师临证、医案整理,并分析传统疼痛病机理论与疼痛致病特点,充分认识方剑乔教授从"瘀""虚"角度阐释慢性疼痛病机的核心内容和主要内涵。[结果]方剑乔教授在传统疼痛病机认识基础上,结合自身丰富的临床经验,首次提出慢性疼痛"瘀虚并存"的致病特点与慢性疼痛"虚瘀交错"病机理论,并总结出了"虚瘀(滞)交错,必先化瘀(滞),结合补虚,方能止痛"的慢性疼痛治疗原则。[结论]方剑乔教授提出慢性疼痛"虚瘀交错"病机理论对于指导疼痛治疗具有重要实际意义,值得推广应用。 [Objective] To summarize professor FANG Jianqiao's novel explanation in neuropathic mechanism of chronic pain and its clinical application,in order to inherit academic ideas of famous TCM doctor. [Method] It was the first time to demonstrate this novel explanation of pain pathogenesis, which was extracted from professor FANG's academic thoughts by interviewing with him, following his guidance in the clinic and collecting his clinical cases,etc. [Result] Based on the traditional theory of pain and combined with his abundant clinical experience, professor FANG demonstrated his novel explanation on pain pathogenesis that meant the innovation of traditional theory. It was considered by professor FANG that no deficient syndrome in pain disease had characterization of pain, pain pathogenesis resulted from interaction of deficiency and stagnation, and also summarized the therapeutic principle to treat painful diseases, exactly, removing stagnation was the primary principle and tonifying deficiency when necessary. [Conclusion] The neuropathic mechanism of chronic pain based on"stagnation"and"deficiency", which demonstrated by professor FANG Jianqiao, is helpful to guide the acupuncturists' clinical experience on acupuncture analgesia and is worthy of inheritance and promotion.
