参麦注射液对重症急性胰腺炎患者细胞免疫功能的影响 被引量:5

摘要 目的探讨参麦注射液辅助治疗重症急性胰腺炎(SAP)患者的临床疗效及其对细胞免疫功能的影响。方法回顾性分析70例SAP患者的临床资料。随机分成观察组(常规治疗+参麦注射液)和对照组(常规治疗),每组35例。比较2组患者的一般资料、临床疗效、治疗前后急性生理学及慢性健康状况评分Ⅱ(APACHEⅡ)及细胞免疫功能指标(NK T细胞、CD_4^+T细胞、CD_8^+T细胞、CD_4^+/CD_8^+)水平及治疗后不良反应发生率。结果 2组患者的一般资料无显著差异(P>0.05)。观察组患者的腹胀腹痛症状消失、体温恢复正常、首次排便、尿及血淀粉酶正常及病程均显著短于对照组(P<0.05)。治疗后2组患者的APACHEⅡ评分均显著低于治疗前,细胞免疫功能各指标水平均显著高于治疗前(P<0.05);治疗后,观察组患者的APACHEⅡ评分显著低于对照组,细胞免疫功能各指标水平显著高于对照组(P<0.05)。观察组治疗后不良反应发生率为5.9%,显著低于对照组的27.3%(χ~2=5.584,P=0.02)。结论参麦注射液辅助治疗可以显著改善SAP患者的细胞免疫功能。 Objective To retrospectively study the influence of Shenmai injection on clinicalefects and cellular immune function of patients with severe acute pancreatitis ( SAP). Methods A total of 70 patients with SAP in our hospital was retrospectively analyzed,and were into observation group (traditional therapy and Shenmai injection) and control group ( traditional thera-py) ,with 35 cases per group. The general information, the clinical effects, acute physiology andchronic health evaluation Ⅱ scores (the APACHE Ⅱ scores) , the immunity indicators (levels of NK T cells,CD4^+ T cells, CD8^+ T cells, CD4^+/CD8^+ ) before and after treatment, the adverse reaction rates in two groups were compared. Results The general information in two significant difference (P 〉0. 05) . The disappearance of abdominal pain and distension,the time thatreturned to normal temperature,the first defection,normal levels of amylase in blood length of stay in observation group were obviously shorter than that in The APACHE Ⅱ scores in two groups after treatment were obviously lower than treathe levels of immunity indicators after treatment were obviously higher than treatment before ( P 〈0. 05) . After treatment, the APACHE Ⅱ score in observation group was obvioumunity indicators in observation group were obviously higher than that in the control group ( P 〈0. 05 ) . The adverse reaction rate in observation group was obviously lower (5.9% vs. 27.3% ,χ^2 =5.584, P =0.02). Conclusion Shenmai injection can improve the cellu-lar immune function of SAP patients.
